A infecção causada pelo vírus da imunodeficiência humana (HIV) acomete indistintamente homens e mulheres, e pode afetar gestantes e/ou seus recém-nascidos (RN). O objetivo foi analisar a infecção pelo HIV como fator de risco para complicações obstétricas e perinatais em gestantes e recém-nascidos atendidos em três maternidades de referências para infecção pelo HIV/aids, no município do Recife, Pernambuco. Estudo epidemiológico do tipo coorte histórica, com base em dados secundários de prontuários de gestantes atendidas no período de 2010 a 2014, foram incluídos dados sociodemográficos e epidemiológicos, clínico-obstétricos e laboratoriais das gestantes e RN. Após serem criticados, os dados foram digitados em plataforma online, por meio de uma ferramenta de sistemas GeneXus, para consolidação e análise. Foram coletadas informações de 1.425 gestantes, sendo 478 infectadas pelo HIV/aids. A análise multivariada da associação entre complicações obstétricas e exposição ao HIV apresentou risco estatisticamente significante apenas para infecção urinária (OR=1,41; IC95%: 1,09-1,83) e ameaça de parto prematuro (OR=1,68; IC95%: 1,17-2,42). Foi verificado efeito protetor para pré-eclâmpsia/eclâmpsia (OR=0,36; IC95%: 0,24-0,56) e corioamnionite (OR=0,13; IC95%: 0,03-0,64). A análise multivariada da associação entre complicações perinatais e exposição ao HIV apresentou risco com significância estatística apenas para perda fetal precoce e óbito perinatal (OR=4,86; IC95%: 1,77-13,39). Para o apgar no primeiro minuto (OR=0,71; IC95%: 0,50-1,02) e CIUR (OR=0,77; IC95%: 0,37-1,58) foi verificado fator de proteção, porém sem significância estatística. Os achados deste estudo corroboram com outros encontrados na literatura. Os resultados apresentados ajudam a refletir sobre a qualidade da assistência prestada às gestantes infectadas pelo HIV/aids e a necessidade de diagnóstico precoce a fim de evitar a transmissão vertical e suas complicações. Infection caused by the human immunodeficiency virus (HIV) affects both men and women, and may affect pregnant women and/or their newborns (NB). The objective was to analyze HIV infection as a risk factor for obstetric and perinatal complications in pregnant women and newborns treated at three maternity hospitals for HIV/AIDS infection in the city of Recife, Pernambuco. Epidemiological study of the historical cohort type, based on secondary data from records of pregnant women attended in the period from 2010 to 2014, included sociodemographic and epidemiological, clinical-obstetric and laboratory data of pregnant women and NB. After being criticized, the data was entered on an online platform, using a systems tool GeneXus, for consolidation and analysis. Information was collected from 1.425 pregnant women, 478 of whom were infected with HIV/AIDS. The multivariate analysis of the association between obstetric complications and exposure to HIV presented a statistically significant risk for urinary infection only (OR=1,41; IC95%: 1,09-1,83) and threat of preterm birth (OR=1,68; IC95%: 1,17-2,42). There was a protective effect for pre-eclampsia/ clampsia (OR=0,36; IC95%: 0,24-0,56) and chorioamnionitis (OR=0,13; IC95%: 0,03-0,64). The multivariate analysis of the association between perinatal complications and exposure to HIV presented a statistically significant risk for early fetal loss and perinatal death (OR=4,86; IC95%: 1,77-13,39). In the first minute apgar (OR=0,71; IC95%: 0,50-1,02) and IUGR (OR=0,77; IC95%: 0,37-1,58), a protection factor was verified, but without statistical significance. The findings of this study corroborate with others found in the literature. The results presented help to reflect on the quality of care provided to pregnant women infected with HIV/AIDS and the need for early diagnosis in order to avoid vertical transmission and its complications. 2019-03-05
