Background The non-nucleoside reverse transcriptase inhibitor (NNRTI), rilpivirine (TMC278; Tibotec Pharmaceuticals, County Cork, Ireland), had equivalent sustained efficacy to efavirenz in a phase 2b trial in treatment-naive patients infected with HIV-1, but fewer adverse events. We aimed to assess non-inferiority of rilpivirine to efavirenz in a phase 3 trial with common background nucleoside or nucleotide reverse transcriptase inhibitors (N[t]RTIs). Methods We undertook a 96-week, phase 3, randomised, double-blind, double-dummy, non-inferiority trial in 98 hospitals or medical centres in 21 countries. We enrolled adults (?18 years) not previously given antiretroviral therapy and with a screening plasma viral load of 5000 copies per mL or more and viral sensitivity to background N(t)RTIs. We randomly allocated patients (1:1) using a computer-generated interactive web-response system to receive oral rilpivirine 25 mg once daily or efavirenz 600 mg once daily; all patients received an investigator-selected regimen of background N(t)RTIs (tenofovir-disoproxil-fumarate plus emtricitabine, zidovudine plus lamivudine, or abacavir plus lamivudine). The primary outcome was non-inferiority (12 margin on logistic regression analysis) at 48 weeks in terms of confirmed response (viral load <50 copies per mL, defined by the intent-to-treat time to loss of virologic response [TLOVR] algorithm) in all patients who received at least one dose of study drug. This study is registered with ClinicalTrials.gov, number NCT00543725. Findings From May 22, 2008, we screened 947 patients and enrolled 340 to each group. 86 of patients (291 of 340) who received at least one dose of rilpivirine responded, compared with 82 of patients (276 of 338) who received at least one dose of efavirenz (difference 3 5 [95 CI -1 7 to 8 8]; pnon-inferiority<0 0001). Increases in CD4 cell counts were much the same between groups. 7 of patients (24 of 340) receiving rilpivirine had a virological failure compared with 5 of patients (18 of 338) receiving efavirenz. 4 of patients (15) in the rilpivirine group and 7 (25) in the efavirenz group discontinued treatment due to adverse events. Grade 2-4 treatment-related adverse events were less common with rilpivirine (16 [54 patients]) than they were with efavirenz (31 [104]; p<0 0001), as were rash and dizziness (p<0 0001 for both) and increases in lipid levels were significantly lower with rilpivirine than they were with efavirenz (p<0 0001). Interpretation Despite a slightly increased incidence of virological failures, a favourable safety profile and non-inferior efficacy compared with efavirenz means that rilpivirine could be a new treatment option for treatment-naive patients infected with HIV-1. The purpose was to identify risk factors associated to deficit on linear growth in children from a semi-rural population in Arandas, Jalisco, Mexico. In a cross sectional study 432 children, 12 to 120 months old were included. Social, demographic, economical, dietetic and pathological characteristics and deficit in the height /age index (< 1 and 2 z score) were considered. A chi square test and Odds Ratio (CI 95%) to identify the risk and protection factors were also obtained. Risk factors for height/age deficit were: > 3 children in the family [OR 1.71 (1.01, 2.87)], soft drinks consumption > 4 times a week [OR 2.36 (1.19-4.73)], family's monthly income < 200 USA dollars [OR 3.5 (1.28, 9.76)] and per capita food expenses < 10% of a minimum salary (100 USA dollars a month) [OR 1.81 (1.06, 3.09)]; age < 24 months [OR 2.02 (1.09, 3.75)], adding sweeteners to the bottle [OR 8.56 (1.84, 54.9)], diet modification during diarrhea [OR 2.40 (1.02, 5.77)], milk intake < 4 times a week [OR 2.71 (1.55.4.73)] and nearly significant, bean consumption [1.75 (0.98, 3.13). Protection factors: nuclear family [OR 0.28 (0.09, 0.85)] and an adequate infant formula dilution [OR 0.71 (0.60, 0.85)]. In multivariate models associated factors to deficit of height were higher consumption of soft drinks, beans and the addition of sweeteners to the bottle. Alejandro, C.",,"1995",,"To assess the potential risk of salmonellosis associated with consumption of chocolate, the prevalence of Salmonella spp. was investigated in retail wrapped and nonwrapped solid chocolate in Guadalajara, Mexico. Previously, the efficiency of preenrichment and enrichment methods to isolate low numbers of Salmonella in chocolate was compared. Though not completely efficient, a preenrichment in nonfat milk without brilliant green, followed by enrichment in tetrathionate and selenite cystine broths gave better results than the other methods studied. Regarding the survey of retail chocolate, Salmonella was isolated from 2 (4.5%) of 44 samples of wrapped chocolate and from none of 56 samples of nonwrapped chocolate. The serovars identified were Salmonella agona and Salmonella derby. There were no significant differences between coliform counts or total bacterial counts obtained from wrapped and nonwrapped chocolate. The presence of Salmonella in chocolate is of concern due to the several low-infectivedose Salmonella outbreaks that have been linked to chocolate. On the other hand, a technique capable of detecting low numbers of Salmonella in chocolate is still needed. Materials and Methods: A cross-sectional study was conducted in patients with histologically confirmed diagnosis of squamous cell carcinoma of the head and neck during 2002-2006. Data on the demographic profile and clinical information were obtained from hospital and clinical records. Staging was based on the American Joint Committee on Cancer and included primary tumor size (T), regional neck status (N), and group stage. The site of cancer was classified based on the International Classification of Disease for oncology (ICD-02). Results: A total of 5458 cases of HNC were included in this study. Majority of the subjects were in the age range of 40-69 years with a significant male preponderance in all the age groups (P<0.001). The most common habit was the combination of smoking, alcohol, and chewing in both males and females (20.1 and 35.1%, respectively) (P<0.001). Tongue and buccal mucosa were the most common sites of cancer in both males (26.8 and 12.8%, respectively) and females (22.9 and 19.8%, respectively) (P<0.001). Tongue was the commonest site of cancer occurrence with respect to all the habits (both singly and in combination) except for chewing tobacco where buccal mucosa was the most common site. Males were more likely to be diagnosed in stage 3 (37.6%) and 4 (20.6%), while females were diagnosed in stage 1 (36.3%) and 2 (32.7%) (P<0.001). Conclusion: A male preponderance of cancer occurrence and combination of all the three habits (smoking, alcohol, and chewing) were found to be the significant risk factors. To assess the risk of dislypidemia associated with obesity in children and adolescents. Material and Methods. A cross sectional study was conducted with 62 obese children (BMI > 95 centile and tricipital skinfold thickness > 90 centile) and 70 non-obese children (BMI 5-85 centile) ages 5-15 years, without chronic diseases. Subjects' characteristics and family background of chronic diseases were collected and a lipid profile was determined. The risk of lipid alterations in the obese children was calculated using odds ratio (OR) and multivariate analysis. Results. Mean age was 9.8 2.7 years in both groups; 63 girls and 69 boys were included. Obesity was associated with abnormal values for cholesterol, triglycerides, LDL, HDL and dislypidemia (> 1 abnormal value) (OR 4.47-15.0). In obese children and adolescents the multivariate analysis showed that female gender was associated with dislypidemia. Conclusion. Clinically he presented normal psychomotor development until 25 months of age when he began with right simple partial motor seizures. He presented minimal dysmorphism, generalized tonic-clonic seizures refractory to medical therapy and behavioral troubles. He was submitted to a callosotomy when he presented an electric status, subsequently, he was treated with anticonvulsivants and felbamate and the seizures were controlled. The karyotype showed a chromosomal complement 46,XY,r(20)(p13q13.3) with loss of the telomeric regions evidenced by FISH. The mother had normal karyotype. The clinical and cytogenetic features of previous cases described in the literature were compared leading to a better characterization of this syndrome.
