In this paper, the structural, sensor, and actuator mathematical models of a transelevator are presented; after, the mathematical model with sensor and actuator of the transelevator is obtained by using the combination of the above mentioned mathematical models. The proposed mathematical model is validated comparing the simulation results against the experimental results. Finally, the stability analysis of the aforementioned model is studied. Hernández-Ibarra, E.",,"2007",,"Chronic diseases are leading causes of morbidity, mortality, and increasing expenditures in numerous countries. However, little is known about how chronic diseases are perceived and managed by social actors. This article aims to compare the perspectives of health professionals and patients towards chronic diseases, besides analyzing the relationship between these two groups. A qualitative, multi-center study was conducted in three Mexican cities: Guadalajara, San Luis Potosi, and Mexico City. Participants included chronically ill individuals, physicians, and other health professionals from primary and secondary health care centers. Data collection used focus groups and interviews. The data were analyzed using discourse analysis. Participants' perceptions varied, from the medicalized view of physicians to that of patients focused on illness and the lifeworld. The participants agreed that there are unequal relationships between health professionals, families, and the chronically ill, but that relationships are more equal among the chronically ill themselves. I.",,"2012",,"OBJECTIVE: Understanding cultural dimensions concerning Mexican and Colombian healthcare workers suffering from dengue to produce information and elements for healthcare and prevention. METHOD: This was a cognitive anthropological study. Purposive sampling was used to select 197 healthcare workers in Morelia, Mexico, and Santiago de Cali, Colombia; free associations lists and pile sorting were used. Terms associated with the concept of dengue and conceptual dimension groups were investigated. RESULTS: Participants cultural conceptions regarding dengue in Mexico emphasised prevention, whilst those in Colombia concentrated on biomedical language and symptoms. Overall, a holistic vision was shown which included a medical vision and related social aspects. Patients with stress incontinence or urgency, local irritation, infection, distal stenosis or a neoplastic process have very similar symptoms. OBJECTIVE: Determine the frequency of bladder structural lesions detected by urethrocistoscopy in which we performed bladder biopsies. MATERIAL AND METHOD: Descriptive, retrospective, analytical study of files and videos of 331 patients treated in Urodifem de Occidente (private Urogynecology Center). Thirty-five biopsies were taken. The statistical analysis was expressed as means standard deviations, ranges, percentages and Fishers test. RESULTS: Patients ages range 30-90 years average 60+13.76. Predominant symptoms were: irritative vesical syndrome 62.8%; pelvic pain 45.71%; urge incontinence 31.4%; hematuria 31.4%; vesical voiding dysfunction 11.4%. Principal endoscopic findings: Urethrotrigonitis; glomerular lesions or Hunner ulcers; vesical trabeculations; tumor or suspect lesions. Histopathologic findings were: Interstitial Cystitis 42.9%; chronic Cystitis 11.4%; Cystitis glandularis 8.6%; Cystitis follicular 11.4%; bladder cancer 5.7%; Vesical Papilloma 5.7%. Preciado-Hernández, F.",,"2006",,"BACKGROUND: The endoscopic placement of endoprostheses to decompress biliary obstruction is a commonly used treatment for malignant biliary diseases and is also used in the treatment of benign biliary strictures. Unusual complications of endoprosthesis placement have been described and include the migration of the stent. We present a case to share with the scientific community, an unusual complication secondary to the migration of a biliary stent that has not previously been reported to our knowledge. CASE REPORT: We present the case of a 47-year-old female with a diagnosis of benign papillary stenosis. The patient received a biliary endoprosthesis with clinical improvement. Later she underwent open cholecystectomy and common duct exploration. At consultation 18 months later, the patient presents with indistinct lower abdominal pain and dysuria. We performed imaging studies where the biliary stent was observed, partly in the sigmoid colon and partly in the bladder. The patient underwent surgery where a colovesical fistula was found and treated during the same surgical event. Calderon-Lara, S.A.",,"2010",,"BACKGROUND: Interstitial cystitis is a disease of unknown origin; in the last twenty years several epidemiological studies reported an increase in frequency. OBJECTIVE: To describe the symptoms, cystoscopic and histologic findings of 18 cases of interstitial cystitis. PATIENTS AND METHOD: A descriptive, retrospective and analytical study of 331 women with lower urinary tract symptoms studied in Urodifem de Occidente, (private Urogynecology Clinic), between January 2001 and April 2008. The diagnostic criterion was in agreement with the NIDDK and the Interstitial Cystitis DataBase Study. The statistical analysis was in interval scale means, standard deviations and ranges. Indeed Spearman's rank correlation coefficient. RESULTS: The most common symptoms were: urinary frequency (100%) nocturia (94.4%), urgency (72%), pain (66.6%), urgency-incontinence (16.7%). Endoscopic lesions were glomerular in 55% and Hunner ulcers in 44.5%. The severity of quality of life resulted in average of 16.7 +/- 2.9 and 15 +/- 2, p < 0.001. CONCLUSIONS: Urogynecologists must considerer interstitial cystitis when patients show symptoms of bladder irritability and associate pain with the bladder filling. The association of hematuria accompanied by long-term irritability and pain associated with the desire of urination suggests this disease. MATERIAL AND METHODS: A total of 157 patients (48 women, and 109 men) from the Departments of Internal Medicine, Gastroenterology and Liver Clinic were studied. Diagnosis of liver cirrhosis had been previously established based on clinical, biochemical or histopathological information. Additionally, a questionnaire specialized on liver diseases was applied to all patients. RESULTS: Alcoholism was the main cause for cirrhosis (38% in women and 95% in men) followed by viral etiology. The alcoholic beverages consumed with greater frequency were tequila and 96 degrees G.L. alcohol. Women were in the Child-Pugh's "B" class while men were in class "C". Complications were mostly gastrointestinal bleeding, ascites and hepatic encephalopathy. Differences between sexes were observed for various characteristics. CONCLUSIONS: Alcoholism was by far the most frequent cause of liver cirrhosis. The second cause, in women, was of viral origin (16.7%). Files of 895 autopsies from 1983 through 1989 by the coroner site office were reviewed. During this seven year period, the annual mortality for drowning was 2.6 per 100,000 population. Males had a higher annual mortality rate (4.2 per 100,000) than females (1.1 per 100,000). The age group between one and four years old had the highest mortality rate (7.6 per 100,000). Deaths tend to cluster around summer. Most of the deaths occurred in house cisterns (19.3%), dams/lakes (16.9%), rivers/canals (14.3%), water wells (12.5%) and swimming pools (10.1%). A third of the deaths occurred at home. The relationship alcohol-drowning starts to stand out in the age group between 10 and 14 years old but get its highest percentage in the age group 35-39 (74%). There exit some validated instruments to assess food and nutrients intake, but there are very few assessing DB. OBJECTIVE: To design and validate a questionnaire to assess DB in Mexican college students. METHODS: According to the literature and Reasoned Theory, a questionnaire assessing DB was designed. Its logic and content validity was determined by expert assessment. It was applied on two occasions with a 4-week interval to 333 students from the University of Guadalajara coursing the sixth semester of Medicine or Nutrition. The reproducibility was assessed by means of the interclass correlation coefficient. The construct validity and the internal consistency were calculated by Rasch analysis, for both the difficulty of the items and the subjects' capability. RESULTS: The questionnaire finally included 31 questions with multiple choice answers. The interclass correlation coefficient of the instrument was 0.76. The Cronbach alpha was 0.50 for the subjects' capability and 0.98 for the internal consistency of the items. 87.1% of the subjects and 89.8% of the items had INFIT and OUTFIT values within acceptable limits. CONCLUSIONS: The present questionnaire has the potentiality of measuring at low cost and in a practical way aspects related with DB in college student with the aim of establishing or following-up corrective or preventive actions.Copyright AULA MEDICA EDICIONES 2014. Homicide rates, adjusted for age, were calculated for both sexes between 1990 and 2009 and for each state in 2008-2009. Factors associated with the interstate variations in the homicide rates were identified using multiple linear regression analysis. Results show that the homicide rate in Mexico decreased between 1990 and 2007, but doubled over the last two years (from 7.6 to 16.6 per 100,000). In 2009, the male homicide rate was almost 9 times higher than the female rate and about two-thirds of homicides involved firearms. Multivariate analysis reveals that impunity, drug trafficking, alcohol and drug consumption and school dropout in basic education - in that order - are key factors for understanding the geographical variations in homicide rates in Mexico in 2008-2009. When zinc concentration decrease, there is a concomitant reduction in insulin secretion and peripheral insulin sensitivity. AIM: To assess the effects of zinc sulfate on insulin sensitivity, leptin and androgens in obese individuals. MATERIAL AND METHODS: A randomized, double-blind, placebo-controlled clinical trial was performed in 14 obese volunteers between 21 and 30 years old, with body mass index (BMI) (3) 27 kg/m2. During one month, seven subjects received 100 mg/day of zinc sulfate orally (ZnG) and the other seven received placebo, as control group (CG). At baseline and after the intervention, insulin sensitivity was measured using a euglycemic-hyperinsulinemic clamp technique. Blood glucose, serum lipids, zinc, androgens and leptin were also measured in a fasting blood sample. RESULTS: After the intervention, a rise in zinc concentrations from 11.8 to 16.9 umol/L; p=0.001 and in leptin levels from 15.2 to 27.7 ng/mL; p=0.029, was observed in the ZnG. No changes were observed in the CG. There were no significant changes in insulin sensitivity and androgens after the intervention with zinc sulfate. G.",,"2012",,"OBJECTIVE: To describe the cultural dimensions that might help or hinder dengue prevention in two Mexican localities having different experiences related to this disease. METHODS: A cognitive-cultural anthropological study was conducted involving the participation of 160 adults selected through purposeful sampling. The techniques of free lists and pile-sorting were used for investigating terms associated with the concept "dengue." A consensus analysis was carried out by the factoring of principal components, hierarchical cluster analysis, and multidimensional scaling. RESULTS: Greater consensus was observed among participants from the population having more historical experience with dengue-Santiago Ixcuintla, Nayarit-as well as more structured cultural and complex dimensions than in Guadalajara. In the latter case, participants showed a more fragmented conception of dengue, with insufficient information on its symptoms, mechanisms of transmission, and vector reservoirs. Three cultural dimensions were identified as hindering dengue prevention: confusing and insufficient information, placing responsibility for prevention on other people or on public agencies, and excessive confidence in fumigation as a preventive measure. Also, both alcohol ingestion and hormonal variations during menstruation have been associated with characteristic changes in electroencephalographic (EEG) activity. AIM. To determine whether EEG activity during a working memory task is affected by acute alcohol consumption, and if these EEG patterns vary in relation to different phases of the menstrual cycle. SUBJECTS AND METHODS: 24 women who drank a moderate dose of alcohol or placebo during the follicular and early luteal phases of the menstrual cycle. The EEG activity was recorded during performance of viso-spatial working memory task. RESULTS: Although the alcohol did not deteriorate the performance of working memory task, it caused in the EEG a decrease of relative theta power and lower right fronto-parietal correlation in theta and alpha2 bands. Only women who drank alcohol in the follicular phase had a higher relative potency of alpha1, which could indicate a lower level of arousal and attention. In Experiment 1, 20 college students received true or false instructions and trial-by-trial or delayed feedback in three phases. In a fourth, final phase the instructions remained the same, but feedback changed from trial-by-trial to delayed, or from delayed to trial-by-trial. In Experiment 2, half of another 20 participants received true instructions during three phases, followed by false instruction in a fourth phase; the other half of the participants received false instructions during three phases, followed by true instructions in the fourth phase. Feedback sequences were as in Experiment 1. The results of both experiments revealed historical effects of instructions and feedback. Most participants demonstrated strong instructional control, overriding the control by contingencies. A clinical assay was carried out in a tertiary care hospital. 10 severely malnourished children < 48 months of age who received formula without lactose via enteral feeding for two weeks and ad libitum for an additional two weeks were included. Cellular and serum concentrations of IL-2 and the subpopulation of CD4+ and CD8+ were obtained. A control group (n = 13) was included. A paired student t test for initial-final determinations and the Mann-Whitney Test for comparison with control group were used, and null hypothesis was rejected with a p value < 0.05. There was a noteworthy increase in the comparison between the initial vs. final percentage of the cellular expression of IL-2 (p < 0.001) and in the serum concentration of IL-2 (p = 0.001). MATERIALS AND METHODS: Mortality records of children younger than 5 years old were obtained from the National Health Information (SINAIS) system of Mexico from 1979 to 2008. Cause of death by asphyxia was established according to the International Classification of Diseases (ICD 9th and 10th). We analyzed age, sex, federal state, year and place where the event occurred. RESULTS: Fatal drowning diminished from 7.64 in 1979 to 3.59 deaths per 100,000 in 2008. This trend was observed throughout the assessment period and in all federal states. Children younger than 2 years showed the highest rate of death. Mortality was higher in males than females (1.7:1). A great proportion of events happen at home. DESIGN: This study is observational, cross-sectional, and prospective. SETTING: The study was carried out in four types of communities in the state of Jalisco, Mexico: migrant, indigenous, rural, and irregular urban communities on the outskirts of cities. PARTICIPANTS: 1277 children under the age of 4 years, who live in the communities selected for the study. PRINCIPAL MEASUREMENTS: Evaluation of the overall development level was based on the Jalisco Scale for Evaluation of Overall Development, which is designed especially for socioeconomically and culturally depressed groups in Mexico. RESULTS: The average development score found in the children was 71.7, which is considered moderately low according to the 3 classification levels of adequate, moderately low, and low. Only 30.4% of the children achieved scores in the adequate category. All the children are considered to live in a high-risk psychosocial environment, which explains the low scores in overall development as evaluated. CONCLUSIONS: Differences were observed in some variables within the uniformly low social level of the subjects studied. These differences could be considered as an indication of greater risk within a group that is already high-risk. In cases of overall development that is very low, mothers do not perceive that children have any problems at the time the study is being conducted. Ethical norms are not enough to appraise the relationship between researchers and participants; a reflexivity practice is needed to understand the dilemmas aroused during this process. Here four issues faced during fieldwork are presented. The academic social time usually defines the schedule of fieldwork; hence, the researcher may decide to follow the academic schedule or to arrange a different schedule with informants. Researchers usually decide which part of their identity will be disclosed for introducing themselves to the informants; but may also decide to hide who really he/she is. Researchers may cope with the dilemma to disclose their private life to informants during the fieldwork. Researchers also may decide to behave according to academic cultural norms of reciprocity or according to the cultural norms of participants. RESEARCH DESIGN AND METHODS: A randomized double-blind clinical trial placebo controlled was performed in 16 obese male volunteers [body mass index (BMI) 30-40 kg/m2]. Eight received orally 1 g of AA daily for four weeks and the other eight volunteers received placebo by the same scheme and period of time. Before and after the pharmacological intervention were measured total cholesterol, high-density-lipoprotein (HDL) cholesterol, triglycerides, glucose, creatinine and uric acid. Low-density-lipoprotein (LDL) cholesterol and very-low-density-lipoprotein (VLDL) triglycerides were calculated using formulas. In order to assess insulin sensitivity before and after the intervention, the steady-state glucose (SSG) was calculated from the insulin suppression test modified with octreotide. RESULTS: There were not significant differences in clinical characteristics between both groups. Basal metabolic profile and SSG were similar between both groups. There were not significant differences in both groups between before and after the intervention in metabolic profile and insulin sensitivity. LV diastolic dysfunction is the first target organ alteration occurring in hypertensive patients, while ventricular hypertrophy is the most relevant predictive factor for cardiovascular morbility and mortality in systemic hypertension. Because several studies have demonstrated that there is no direct correlation between blood pressure values and the severity of LV hypertrophy or diastolic dysfunction, it is assumed that other factors are involved in the genesis of these functional alterations. Moreover, the hypertensive effect of insulin is caused by sympathetic stimulation, sodium and water renal retention and protooncogene stimulation leading to myocardial and vascular fibrosis and hypertrophy. We studied 24 hypertensive patients with hyperinsulinemia. All patients underwent an overall and cardiologic clinical evaluation, and electrocardiographic and ecocardiographic studies were performed at baseline and 6 months after being treated with 2.5 to 5 mg/day ramipril. Ramipril treatment significantly reduced systolic (12 mmHg) and diastolic (12 mmHg) pressure levels, basal insulin serum levels (23.62 pmol/dL vs 10.42 pmol/dL), and left ventricle mass index values (143.8 g/m2 vs 118.2 g/m2). Among the variables assessing LV diastolic function, only the transmitral flow E/A wave ratio showed significant differences in women. The treatments were applied under "seed" trees, and the number of germinated seedlings and their development were evaluated for nine months. Soil "scarification" promoted seed germination and initial seedling development, while the control of the competing vegetation increased the seedling growth and reduced their mortality. The impact of the Sanitary Normativity in the seroprevalence of anti HIV antibodies between blood donors one year before and one year after it took result was compared. A sample was taken from 4743 serums of donors of 6 blood banks in Guadalajara between june 1986 and june 1988; 4020 in the first year and 723 the next. In all the samples the presence of anti HIV antibodies was determined by the ELISA assay and the positive ones were confirmed by indirect immunofluorescence and by Western blot. During the first year, the seroprevalence fluctuated between 2.02 and 30.15 percent depending on different factors, differing from the second year where the seroprevalence was 0.14 percent. OBJECTIVE: To evaluate health related quality of life in non-Hodgkin lymphoma and colorectal cancer cases in different clinical stages, attended in a High Specialty Medical facility at the Instituto Mexicano del Seguro Social, during a 13 month period. RESULTS: 162 patients were included, 56.8% (n=92) with NHL and 43.2% (n=70) with colorectal cancer. The scores obtained in the NHL group were: Global health status/QoL: 67.75 (+/- 27.55), physical functioning 69.64 (+/- 29.98), role functioning 71.38 (+/- 33.73), emotional functioning 69.7 (+/- 26.57), cognitive functioning 75.36 (+/- 28.01), social functioning 79.35 (+/- 29.38), fatigue 35.27 (+/- 28.27), nausea and vomiting 13.41 (+/- 21.85), pain 28.08 (+/- 30.25), dyspnea 19.20 (+/- 32.11), insomnia 30.80 (+/- 38.03), appetite lost 26.45 (+/- 36.16), constipation 19.20 (+/- 32.11), diarrhea 12.32 (+/- 26.48), financial difficulties 26.09 (+/- 35.57). In colorectal cancer patients the scores were: Global health status/QoL: 68.21 (+/- 24.46), physical functioning 67.38 (+/- 30.45), role functioning 65.48 (+/- 35.70), emotional functioning 66.43 (+/- 26.84), cognitive functioning 78.57 (+/- 26.49), social functioning 75.24 (+/- 31.05), fatigue 37.78 (+/- 31.62), nausea and vomiting 20.00 (+/- 28.32), pain 37.38 (+/- 34.45), dyspnea 11.90 (+/- 26.64), insomnia 28.09 (+/- 35.73), appetite lost 23.81 (+/- 36.40), constipation 19.05 (+/- 32.88), diarrhea 20.95 (+/- 31.17), financial difficulties 34.76 (+/- 38.67).
