Bichectomy is a therapeutic resource of increasing interest in the surgical head and neck area that consists in the removal of cheek fat pad, known as the Bichat Ball (BB), located between the buccinator and masseter muscles. Its utility is still discussed in the literature. The surgery has some objectives that are to improve the aesthetics of the face, improving harmony smile, as well as the reconstructive function of defects in the head and neck region. The use of imaging diagnostics, notably in ultrasound, allows that relevant information about BB's location and anatomy, as well as its relation with adjacent anatomical structures. This information makes the surgical planning of the procedures safer and predictable. This paper assess essays on surgical procedures that involve the removal of BB, including those related to diagnostic methods for the planning of bichectomy, for the purpose of evaluating changes in the patient's aesthetic and functional patterns, defining a description of their instructions, treatment and prognosis of patients undergoing this procedure.A bichectomia é um recurso terapêutico que consiste na remoção do tecido adiposo da bochecha denominada Bola de Bichat (BB). Localizada entre os músculos bucinador e masseter, a BB possui funcionalidade indiscutível fisiologicamente e na aplicabilidade na área da cirurgia de cabeça e pescoço. Atualmente cresce o interesse estético na bichectomia, pois ela é capaz de conferir melhora estética da face e do sorriso. O presente trabalho tem como objetivo a revisão da literatura acerca dos procedimentos cirúrgicos que envolvem a remoção do tecido adiposo bucal, tendo como coadjuvante os exames de imagem, principalmente a ultrassonografia para o planejamento e prognostico através da localização das estruturas nobres que envolvem a BB. Além disso é necessário avaliar a modificação do padrão estético e funcional do paciente a fim de indicar ou não a sua remoção.
