Os Centros de Testagem e Aconselhamento (CTA) são serviços de diagnóstico de infecções sexualmente transmissíveis (IST), especialmente de HIV, sífilis e hepatites virais. Em Pernambuco, do total de diagnósticos de HIV realizados no ano de 2015, ocorreram nos CTA 61,8% na população geral e 57,8% em gestantes. A multiplicidade de formas dos CTA e os notáveis problemas da Vigilância Epidemiológica (VE) motivaram a realização desta análise de implantação. Para tanto, foram elaborados: um modelo lógico; uma matriz de análise e julgamento, com os escores: implantada (≥80%); parcialmente implantada (79 a 60%); insatisfatória (59 a 40%); e não implantada (<40%); e uma matriz de contexto, com um questionário semiestruturado a partir das bases do “Triângulo de Governo de Matus”, classificando o contexto, como: favorável (>66,6%); pouco favorável (≤66,6% a >33,3%); e não favorável (≤33,3%). Os resultados demonstraram que as ações de VE, obtiveram níveis de implantação insatisfatórios, tanto no geral (51,5%), quanto nos dois componentes analisados (Organização 51,4% e Processo de Trabalho 51,7%). Quanto ao contexto organizacional, as três categorias do triângulo “Matusiano” foram pouco favoráveis à implantação das ações de VE nos CTA, o que ratifica a interdependência entre os seus vértices e a coerência entre os aspectos observados na conjuntura e o nível de implantação das ações de VE. Concluindo, o presente estudo evidenciou que o contexto tem influenciado na vigilância do HIV/aids nos CTA. Além disso, a multiplicidade organizacional, sofrida ao longo dos anos, têm distanciado os CTA de sua principal missão “Atividades de Prevenção e Vigilância das IST”, apontando a necessidade de investimentos na reestruturação desses serviços.The Testing and Counseling Centers (TCC) are diagnostic services for sexually transmitted infections (STI), especially HIV, syphilis and viral hepatitis. Therefore, were elaborated: a logical model; a matrix of analysis and judgment, with scores: implanted (≥80%); partially implanted (79 to 60%); unsatisfactory (59 to 40%); and not implanted (<40%); and a context matrix, with a semi-structured questionnaire based on the "Matus Government Triangle", classifying the context as: favorable (> 66,6%); little favorable (66,6% to >33,3%); and not favorable (≤33,3%). The results demonstrated that ES actions achieved unsatisfactory levels of implantation, both in general (51,5%) and in the two components analyzed (Organization 51,4% and Work Process 51,7%). About the organizational context, the three categories of the "Matusian" triangle were little favorable to the implantation of ES actions in the TCC, which ratifies the interdependence between its vertices and the coherence between the observed aspects in the conjuncture and the level of implantation of the actions of ES. In conclusion, the present study demonstrated that the context has influenced the surveillance of HIV/aids in TCC. In addition, the organizational multiplicity suffered over the years has distanced the TCC from its main mission "Prevention and Surveillance Activities of STI", pointing out the need for investments in the restructuring of these services.2018-04-23
