dc.descriptionEste trabalho teve como objetivo geral propor a adaptação do enfoque do Planejamento Estratégico Situacional (PES) para o nível local em saúde considerando a abordagem comunicativa e a dimensão da equidade. Para tanto, além do referencial teórico o trabalho apoiou-se na aplicação do enfoque em uma das paróquias do Estado Aragua, Venezuela a partir de uma problemática em saúde priorizada pelos atores-chave e que resultou num plano de ação. Se realizou um estudo documental e de campo. A seleção da paróquia foi realizada a partir de dois critérios considerados inovadores na aplicação do PES: (a) a análise para a estimação das necessidades relativas de saúde; (b) a análise do potencial participativo dos atores-chave frente às operações do plano. Ficou selecionada a paróquia Capital Santos Michelena do Estado Aragua, onde os atores priorizaram, entre vários dos problemas existentes, a “baixa capacidade resolutiva da rede de serviços de saúde”. Nos três momentos do PES aplicados na paróquia para o processamento do problema utilizaram-se diversas técnicas metodológicas. Destaca-se no momento explicativo: a) a identificação de problemas, relacionando-os com os artigos referentes a direitos consagrados na Constituição venezuelana, b) a conformação de territórios sociais a partir da utilização de um guia de observação e acompanhamento com informantes chaves de cada localidade; c) a medição de iniqüidades a partir de indicadores de saúde associados ao problema selecionado. No momento normativo foram incorporados critérios de equidade em cada operação, fundamentalmente considerando a análise das brechas por iniqüidade nos territórios sociais. No momento estratégico, a análise da motivação dos atores-chave em face às operações do plano de ação e a análise da capacidade de ação (poder), procuraram ressaltar elementos comunicativos, como a negociação e consideração de atores com interesses divergentes não como oponentes, mas desde o início como colaboradores potenciais. Da análise do problema foram selecionados cinco nós críticos para desenhar o plano de ação. Conformaram-se os territórios sociais através da construção de um indicador ponderado e dos 42 territórios sociais da paróquia Capital Santos Michelena, 38 foram classificados nas categorias: condições de vida regulares, condições de vida ruins e condições de vida muito ruins. Se analisou a presença de iniqüidades entre os extratos sociais mediante o cálculo da ANOVA para indicadores relacionados com a cobertura dos serviços de saúde. O plano de ação ficou constituído por 6 operações e 21 ações, das quais duas são ações de demanda para Corposalud Aragua e a prefeitura do município. A conclusão principal deste estudo é que a incorporação do enfoque comunicativo e da dimensão da equidade no PES permite potencializar a gestão em saúde e contribuir para a diminuição das brechas por iniqüidade.
dc.descriptionThis study had like general objective proposed the adaptation of a Situational Planning Strategy (SPS) for the local health level that considers the communicative boarding and the dimension of the equality. For so much, besides the theoretical referential system the work supported in the application of the approach in one of the parishes of the Aragua State, Venezuela from a problematic in health prioritized by the actors-key and what turned in a plan of action. A documentary study and of field was made. The selection of the parish was carried out from two criteria considered innovatory in the application of SPS: (a) The analysis for the estimation of the relative necessities of health and; (b) The analysis of the participative potential of the actor-key at front of the operational plan. The selected area was the Santos Michelena’s Capital Parrish where the actors prioritized, in between of several existing problems, the “low solving capacity of the local health system net”. At three moments of SPS applied in the parish for the processing of the problem were used diverse methodology techniques. It stands out at the explicative moment: a) the identification of problems connecting them with the articles referring to right consecrated in Constitution Venezuelan, b) the construction of social territories from the use of the Observational Guide with support of key-informants of each area; c) measurement of iniquities based on health indicators associated with the problem select. At the strategic moment criteria of equity were incorporated in each operation, fundamentally considering the analysis of the breaches by iniquity in the social territories. At the strategic moment, the analysis of the actor-key motivation in front of the plan of action and the analysis of the capacity for the action (power), tried to emphasize communicative elements, like the negotiation and actors' consideration with divergent interests not eat opponents, but from the beginning like potential collaborators. From the analysis of the problem five critical aspects were selected to design the plan of action. The social territories were constructed through the conformation of a pondered indicator and from the 42 areas that make the Santos Michelena’s Parrish, 38 were classified into the following categories: regular life conditions, bad life conditions and very bad life conditions. Similarly, the analysis of iniquities in the different social strata within the population was made based on the calculus of ANOVA, which is related to the coverage of the health services. The plan of action was constituted by 6 operations and 21 actions, of which two are actions of demand for Corposalud Aragua and the town hall of the local authority. It is the principal conclusion of this study what the incorporation of a communicative approach allows the improvement and reinforcement of the health and of the dimension of the equity in allows SPS to strengthen the management in health and to contribute to the reduction of the iniquities within the population.
