Abstract

The knee joint is a complex structure and may be susceptible to the involvement of different types of injuries. Since most of these injuries require a costly, long and continuous treatment, diagnostic methods are essential for the correct identification and to justify the treatment. Thus, classification systems, flowcharts, and questionnaires for the assessment of the functional condition and the establishment of the degree of severity of the lesion are widely important in directing and monitoring the prognostic evolution. Objective: The main objective of this work is to develop an updated national technical reference, with the various classifications and flowcharts in knee surgery, which will serve as a source of information and consultation for orthopedists, specialists, and specialists in knee surgery. Materials and methods: The present work is a literature review and subsequent creation of a book with classifications and flowcharts in knee surgery. As reference bibliographies, the main books adopted for this approach and a wide systematic search in journals indexed in the databases were used. As criteria and inclusion, classifications and flowcharts were established in reference bibliographies and indexed scientific articles. And, by exclusion, tumor lesions involving the knee. Results: After applying the determined inclusion and exclusion criteria, a total of 117 classifications, 36 flowcharts, and 6 functional questionnaires were included in the book entitled “Classifications and Flowcharts in Knee Surgery”. To facilitate the organization and facilitate the consultation, the classifications were organized in specific chapters by sub-themes. They are: Meniscal Injuries; Chondral/patellofemoral lesions; Ligament Injuries; Fractures; Musculotendinous Injuries; Degenerative Lesions. The book was designed in such a way as to favor the reader's easy identification and interpretation. Discussion: The complexity of the knee joint and the numerous criteria to assess its function make it difficult to measure and quantify the diagnostic techniques and the possibilities of treatments used. Thus, classifications play a fundamental role in knee surgery and can help professionals through complete cataloging of treatment approaches, prognostic estimates, and standardization of nomenclatures, which are essential for the development of an effective protocol. Conclusion: The book “Classifications and Flowcharts in Knee Surgery” is a national reference work directed to knee surgery. The book covers and brings together the various classifications, flowcharts, and questionnaires in the area, helping in continuing education, in the preparation of specialists, and the updating of specialists.

A articulação do joelho é uma estrutura complexa, podendo ser suscetível ao acometimento de diferentes tipos de lesões. Uma vez que, a maioria dessas lesões exigem um oneroso, longo e contínuo tratamento, métodos de diagnósticos se fazem essenciais para a correta identificação e fundamentação do tratamento. Assim, sistemas de classificações, fluxogramas e questionários para a avaliação da condição funcional e o estabelecimento do grau de severidade da lesão, são amplamente importantes no direcionamento e acompanhamento da evolução prognóstica. Objetivo: O objetivo principal deste trabalho é elaborar uma referência técnica nacional atualizada, com as diversas classificações e fluxogramas em cirurgia do joelho, que servirá como fonte de informações e consulta para ortopedistas, especialistas e especializandos em cirurgia do joelho. Materiais e métodos: O presente trabalho trata-se de uma revisão da literatura e posterior confecção de um livro com classificações e fluxogramas em cirurgia do joelho. Como bibliografias de referência foram utilizados os principais livros adotados para esta abordagem e uma ampla pesquisa sistemática em periódicos indexados nas bases de dados. Como critérios de inclusão, estabeleceu-se classificações e fluxogramas nas bibliografias de referência e em artigos científicos indexados. E, de exclusão, lesões tumorais envolvendo o joelho. Resultados: Após a aplicação dos critérios de inclusão e exclusão determinados, um total de 117 classificações, 36 fluxogramas e 6 questionários funcionais foram incluídos no livro intitulado “Classificações e Fluxogramas em Cirurgia do Joelho”. Para facilitar a organização e propiciar a eficácia da consulta, as classificações foram organizadas em capítulos específicos por subtemas. Sendo eles: Lesões Meniscais; Lesão Condral/Femoropatelar; Lesões Ligamentares; Fraturas; Lesões Musculotendíneas; Lesões Degenerativas. O livro foi diagramado de forma a privilegiar a fácil identificação e interpretação do leitor. Discussão: A complexidade da articulação do joelho e os numerosos critérios para avaliar sua função, tornam difícil mensurar e quantificar as técnicas diagnósticas e as possibilidades de tratamentos empregados. Deste modo, as classificações desempenham um papel fundamental na cirurgia do joelho, podendo auxiliar o profissional através de uma completa catalogação de condutas de tratamento, estimativas do prognóstico e padronização de nomenclaturas, imprescindíveis para elaboração de um protocolo eficaz. Conclusão: O livro “Classificações e Fluxogramas em Cirurgia do Joelho” é uma obra de referência nacional direcionada à cirurgia do joelho. O livro abrange e reúne as diversas classificações, fluxogramas e questionários da área, auxiliando na educação continuada, na preparação de especializandos e na atualização dos especialistas.
