Coombs test and its relationship with neonatal jaundice in the neonatal jaundice at the Dr. Verdi Cevallos Balda Hospital between January and March 2021

Neonatal jaundice is clinically detected when the newborn presents a yellowish coloration of the skin and mucous membranes caused by the deposit of bilirubin, this occurs when the bilirubin exceeds the figure of 5 mg/dL. When bilirubin does not quickly return to normal values, medical concern begins to find out the origin of this situation. An important cause of neonatal jaundice is maternal-fetal incompatibility and for this some diagnostic tests are requested, including the Coombs test. The objective of this research is to evaluate the relationship between neonatal jaundice and the Coombs test at the Dr. Verdi Cevallos Balda Hospital. The study was observational, retrospective, descriptive and cross-sectional, analyzing data from the area of Neonatology, Transfusion Medicine and Statistics. Neonatal jaundice was determined in 49 neonates born between January and March 2021. Of these 49 neonates, 39 presented a positive direct Coombs test (79.59%). A significant association was determined between both variables p=0.000. 66.67% of the neonates presented blood group B and 98% were Rh positive. The high number of positive Coombs tests in neonates is related to cases of fetal-maternal incompatibility as one of the main causes of jaundice in said hospital. La ictericia neonatal clínicamente se detecta cuando el recién nacido presenta una coloración amarillenta de piel y mucosas ocasionada por el depósito de bilirrubina, esto ocurre cuando la bilirrubina sobrepasa la cifra de 5 mg/dL. Cuando la bilirrubina no retorna rápidamente a los valores normales, comienza la preocupación médica de conocer el origen de esta situación. Una causa importante de ictericia neonatal es la incompatibilidad materno-fetal y para ello se solicitan algunas pruebas diagnósticas, entre ellas el test de Coombs. El objetivo de esta investigación, radica en evaluar la relación entre la ictericia neonatal y el test de Coombs en el Hospital Dr. Verdi Cevallos Balda. El estudio fue observacional, retrospectivo, descriptivo y de corte transversal, analizando los datos del área de Neonatología, Medicina Transfusional y Estadística. Se determinó ictericia neonatal en 49 neonatos nacidos entre enero y marzo 2021. De estos 49 neonatos, 39 presentaron un test de Coombs directo positivo (79,59%). Se determinó asociación significativa entre ambas variables p=0.000. El 66, 67% de los neonatos presentó grupos sanguíneo B y 98% era Rh positivo. El elevado número de test de Coombs positivos en los neonatos, se relaciona con casos de incompatibilidad materno fetal como una de las principales causas de ictericia en dicho centro hospitalario.
