Resumen
Objetivo: Determinar la asociación entre aterosclerosis detectada por ultrasonido y factores de riesgo cardiovascular en los pacientes con lupus eritematoso sistémico (LES) en el Servicio de Reumatología del Hospital Nacional Dos de Mayo Material y métodos: Se incluyó pacientes con diagnóstico de LES entre enero a marzo del 2009 a quienes se les realizó ultrasonido de carótidas para medir el grosor de la íntima-media y determinar la presencia de placa aterosclerótica. Se describió características clínicas, tratamiento y factores de riesgo clásicos para ateroesclerosis. Se calculó OR e intervalo de confianza con un nivel de significancia estadística del 95%.Las comparaciones fueron determinadas mediante la prueba del chi cuadrado. Resultados: Fueron evaluados 40 pacientes, el promedio de edad fue de 31 años y la distribución por sexo fue predominantemente femenino (90%). La prevalencia de placa aterosclerótica evidenciada por ultrasonografía fue de 17,5%. Dentro de los factores de riesgo asociados a la presencia de placa aterosclerótica tenemos: el sobrepeso (odds ratio [0R]: 2,051; intervalo de confianza [IC] del 95%: 0,393-10.7), hipertensión sistólica (OR: 2,9; IC: 0,29-97,48), y la diabetes mellitus (OR: 5,33; IC: 0,29-97,48) demostraron un riesgo de asociación. Las dislipidemias no presentaron asociación. En cuanto a compromiso sistémico no hubo diferencia significativa entre ambos grupos. Tampoco con la puntuación del Systemic Lupus Erythematosus Disease Activity Index (SLEDAI). Conclusiones: Identificamos algunos factores de riesgo tradicionales asociados a la presencia de placa ateroesclerótica tales como la hipertensión, diabetes mellitus, sobrepeso. Trabajo académico
