Hypertension is a chronic disease determined by high levels of blood pressure in the arteries, which causes the heart has to exert more effort than normal to circulate blood through the blood vessels, in turn, high blood pressure It is a risk factor for coronary heart disease, cerebrovascular disease, peripheral vascular disease, heart failure and end stage renal disease. These diseases are major causes of morbidity and mortality, with high social cost. In response to the assumptions of the Family Health Program (PSF) was developed intervention project in line with the current promotion and protection of health policies in the educational perspective, since every change requires an educational process. This project aims to develop an intervention plan to act on the risk factors for hypertension among the population in the Family Health Strategy Our Lady of Grace in the municipality of Divinópolis. To prepare the action plan we used the Strategic Planning Method Situational. The proposed action plan will direct a team of professionals in service to hypertensive patients to improve knowledge of these individuals about the disease, provide Vidam quality improvements and significantly reduce modifiable risk factors for high blood pressure.
