dc.description.abstract | Introduccion: La educación médica en Colombia lleva más de 2 siglos de historia, ha
venido evolucionando y adaptándose a los cambios propios de la modernización. En todo
el mundo, la adecuada educación del residente quirúrgico ha requerido en promedio de 5
años. La educación de residentes quirúrgicos basada en competencias promueve la
adquisición de actitudes y aptitudes en ambientes de aprendizaje controlados, siendo la
simulación quirúrgica una herramienta fundamental. La evaluación objetiva de la habilidad
quirúrgica, con escalas como la OSATS, ha demostrado mejoras significativas en las
habilidades de los participantes. El objetivo del presente estudio es evaluar las habilidades
laparoscópicas simuladas de residentes de cirugía general utilizando la escala OSATS en
simuladores de mesa.
Metodología: Estudio de corte transversal, se incluyeron residentes de cirugía general de
primer hasta cuarto año y profesores e instructores de cirugía general de la UNAL en los
años 2021-2022. Se aplicó una prueba de 5 ejercicios en un simulador de mesa y dos
evaluadores cegados calificaron cada ejercicio con 5 items de la escala OSATS. Se realizó
un análisis descriptivo de los perfiles de los participantes, se calculó el puntaje total
promedio por perfil, se realizó un ANOVA de medidas repetidas para los análisis
multivariados.
Resultados: Se incluyeron 34 participantes, de los cuales 12 eran profesores, 6 residentes
de primer año, 6 de segundo año, 5 de tercer año y 5 de cuarto año. Participaron 2
evaluadores. Los residentes de cuarto año tuvieron el mayor puntaje promedio 14.28 (DE
2.54) mientras que los residentes de primer año tuvieron el menor puntaje promedio 7.40
(DE 3.27). En la prueba de ANOVA de medidas repetidas encontramos que el nivel de
formación se asocia con el puntaje obtenido en la prueba con una p<0.01. Adicionalmente,
los residentes de segundo año tuvieron mayor puntaje promedio 13.96 (DE 2.56) con
respecto a los residentes de primer año que tuvieron un puntaje promedio 7.40 (DE 3.28).
En la prueba de ANOVA de medidas repetidas encontramos que el nivel de entrenamiento
formal en laparoscopia se asocia con un mejor puntaje obtenido en la prueba con una
p<0.01.
Conclusiones: Por lo menos en nuestro programa de residencia, hay una asociación entre
el mayor grado de formación en residentes y mejores habilidades laparoscópicas
simuladas. Adicionalemne, hay una asociación entre el entrenamiento formal en simulación

dc.description.abstract | Introduction: Medical education in Colombia has more than 2 centuries of history, it has
been evolving and adapting to the changes of modernization. Throughout the world, the
adequate education of the surgical resident has required an average of 5 years. The
competency-based education of surgical residents promotes the acquisition of attitudes and
aptitudes in controlled learning environments, surgical simulation being a fundamental tool.
The objective evaluation of surgical skill, with scales such as the OSATS, has shown
significant improvements in the skills of the participants. The objective of the present study
is to evaluate the simulated laparoscopic skills of general surgery residents using the
OSATS scale on tabletop simulators.
Methodology: Cross-sectional study, general surgery residents from the first to fourth year
and general surgery professors and instructors from UNAL in the years 2021-2022 were
included. A test of 5 exercises was applied on a tabletop simulator and two blinded
evaluators rated each exercise with 5 items from the OSATS scale. A descriptive analysis
of the profiles of the participants was performed, the average total score per profile was
calculated, and an ANOVA of repeated measures was performed for the multivariate
analyses.
Results: 34 participants were included, of which 12 were teachers, 6 first-year residents, 6
second-year residents, 5 third-year residents, and 5 fourth-year residents. 2 evaluators
participated. Fourth-year residents had the highest mean score of 14.28 (SD 2.54) while
first-year residents had the lowest mean score of 7.40 (SD 3.27). In the ANOVA test of
repeated measures, we found that the level of training is associated with the score obtained
in the test with a p<0.01. Additionally, second-year residents had a higher mean score of
13.96 (SD 2.56) compared to first-year residents who had a mean score of 7.40 (SD 3.28).
In the repeated measures ANOVA test, we found that the level of formal training in
laparoscopy is associated with a better score obtained in the test with a p<0.01.
Conclusions: At least in our residency program, there is an association between a higher
degree of resident training and better simulated laparoscopic skills. Additionally, there is an
association between formal simulation training and better simulated laparoscopic skills. | |
