Factores preoperatorios asociados a la conversión de colecistecomía laparoscópica a colecistectomía abierta en el Hospital Sergio E. Bernales de enero 2013 a diciembre 2016

Antecedentes: La patología biliar, sobre todo colelitiasis, es muy prevalente y su manejo generalmente requiere realizar la colecistectomía laparoscópica. En ocasiones, ésta debe convertirse a colecistectomía abierta, asociándose una mayor morbilidad. Diversas investigaciones han identificado los factores preoperatorios asociados a dicha conversión, como demográficos, clínicos, laboratoriales, imagenológicos y diagnósticos preoperatorios. No obstante, su preponderancia individual de éstos varía grandemente de un estudio a otro.
Método: El presente es un estudio epidemiológico, observacional, analítico, de casos y controles que busca identificar los factores preoperatorios asociados a la conversión quirúrgica en los pacientes atendido en el Hospital Nacional Sergio E. Bernales de enero 2013 a diciembre 2016.
Resultados: Se realizaron 2,917 colecistectomías laparoscópicas durante el periodo de estudio, de las cuales 894 eran plausibles de revisión. Entre éstas se identificaron 84 conversiones a colecistectomía abierta, con una tasa de conversión de 9.4%. Los factores preoperatorios asociados fueron el sexo masculino (OR = 2.0; IC95% 1.0 - 3.8; p < 0.05), la obesidad (OR = 2.6; IC95% 1.0 - 6.7; p < 0.05), bilirrubina total > 1.2 mg/dl (OR = 2.4; IC95% 1.3 - 4.5; p < 0.01), recuento de leucocitos > 12,000/mm3 (OR = 1.9; IC95% 1.0 - 3.4; p < 0.05) y un diagnóstico preoperatorio de colecistitis crónica reagudizada (OR = 2.0; IC95% 1.0 - 3.7; p < 0.05).
Conclusiones: Los factores asociados a conversión quirúrgica fueron el sexo masculino, la obesidad, hiperbilirrubinemia, leucocitosis y diagnóstico preoperatorio de colecistitis crónica reagudizada.
