Síndrome metabólico asociado a mayor daño en pacientes con Lupus Erimatoso Sistémico del Hospital Guillermo Almenara Irigoyen entre enero 2012 enero 2020

Objetivos: Determinar la asociación entre síndrome metabólico (SM) y mayor daño orgánico en pacientes con lupus (LES). Materiales y métodos: evaluamos a 314 pacientes, miembros de la Cohorte de Lupus del Almenara, durante enero 2012 a enero 2020. Aplicamos los criterios de: International Diabetes Fedetation/American Health Association/National Heart, Lung and Blood Institute para determinar SM; el índice de daño de Systemic Lupus International Collaborating Clinics/American College of Rheumatology para determinar daño; y Systemic Lupus Erythematosus Disease Activity Index para evaluar la actividad. Realizamos un análisis univariado y multivariado usando ecuaciones de estimación generalizada, ajustado para potenciales confusores (factores demográficos, factores relacionados a la enfermedad y tratamiento). El análisis estadístico se realizó en el programa SPSS v.28.0. Resultados: Se incluyeron 314 pacientes y 1695 visitas, el 26.2% de pacientes tuvo SM. Entre los componentes de SM, destacaron la obesidad central (76.1%), seguido de hipertensión (40.5%), HDL disminuidos (39.2%), triglicéridos elevados (31.4%) y diabetes (11.1%). En el análisis multivariado, no hubo asociación de SM con mayor daño en LES (OR: 1.143; 0.818-1.597, P=0.432). la hipertensión se asoció a mayor daño (OR: 1.645; 1.187-2.280, P=0.003). Obesidad central se asoció con menos daño (OR: 0.637; 0.441-0.920, P=0.016). Los otros componentes no se asociaron con daño: HDL disminuidos (OR: 0.812; 0.595-1.109, P=0.191), triglicéridos elevados (OR: 1.087; 0.781-1.511, P=0.621) y diabetes (OR: 0.984; 0.649-1.492, P=0.939). Conclusión: el SM no se asoció a mayor daño en LES. La hipertensión se asocia al incremento el daño. Debida a la alta prevalencia del SM, debería incluirse en la evaluación rutinaria de pacientes lúpicos.
