Resumen
El presente trabajo de investigación va a describir la planificación y
tratamiento quirúrgico de las cirugías preprotésicas del territorio maxilofacial
en una revisión bibliográfica más no experimental. Se realizó una
investigación basada en recopilación de datos bibliográficos, describiendo
todo lo referente al tema.
Se engloba en el término de cirugía preprotética, a todas aquellas
intervenciones quirúrgicas destinadas a mejorar o condicionar los maxilares,
en especial a los procesos alveolares, con el fin de adaptar una prótesis
dental, funcional y estéticamente cónsona con la normalidad. Su finalidad es
la de obtener un proceso alveolar bien conformado, de dimensiones
normales con los respectivos surcos tanto vestibular como el glosofaríngeo
de una profundidad adecuada y paredes paralelas y de borde superior lo más
cercano a la horizontal. Su mucosa debe ser adherente no muy gruesa, pero
tampoco delgada y su mucosa libre de su base bien elástica y carente de
inserciones musculares y bridas cicatrízales.
Estas necesidades surgen porque el paciente desdentado presenta unos
problemas derivados de sus condiciones anatómicas alteradas y estos
problemas se acentúan en el caso de edentulismo total agudizándose más
aun cuando el paciente es de edad avanzada. Son muchos los pacientes con
antecedentes bucales preprotéticos que acuden al servicio de prótesis
necesitados de tratamientos, a los que después del examen clínico intrabucal
es preciso remitir nuevamente al servicio de cirugía para mejorar el lecho
biológico por insuficiencia del tratamiento. En cuenta lo planteado por Zaizar,
"toda extracción debe considerarse como cirugía preprotésica", podemos
afirmar que toda la cirugía oral menor está en relación con la prótesis. Cada
vez que se opera, debe considerarse como muy probable, sino seguro, que en un futuro próximo o lejano una prótesis tomará contacto con la región
intervenida, por lo que siempre se deben elegir los tratamientos quirúrgicos
menos mutilantes, sea evitando, eliminando o corrigiendo aquellas
disposiciones anatómicas que dificulten o impidan la colocación correcta de
una prótesis.
Existen varias clasificaciones de la cirugía preprotésica. Según Archer se
divide en las anomalías que afectan tejidos blandos, óseos y mixtos. Kruger
señala que se debe tener en cuenta el momento de la cirugía clasificándola
en preparaciones iniciales y preparaciones secundarias. Como se ha podido
constatar, la cirugía preprotésica constituye un acápite importante de
investigación, pues son muchos los pacientes que acuden diariamente a
nuestros servicios con estas alteraciones, nos motivó a realizar esta
investigación, que sirva de base para futuros estudios, además para la
organización del trabajo docente asistencial y la planificación de los recursos
humanos y materiales en los servicios como contribución al ahorro. This research paper will describe the planning and
surgical treatment of maxillofacial surgeries preprosthetic
in a non-experimental literature review. He made an
research based on bibliographic data collection , describing
everything about the subject.
Is included in the term Preprosthetic surgery, all those
surgical procedures or condition to improve the jaws,
especially the alveolar process , in order to adapt a prosthesis
dental , functional and aesthetically in harmony with normal. Its purpose is
