La Operatoria Dental en el pasado se desarrolló empíricamente, pues con
la civilización, se inició la caries dental, por tanto el hombre debe haber
buscado desde tiempos muy primitivos el alivio a las molestias
ocasionadas por la caries.
Ya en excavaciones realizadas en el antiguo Egipto se ha encontrado
momias con relleno de oro en sus dientes tallados. Son estas las primeras
obturaciones encontradas, lo que no se ha logrado saber es si estas
obturaciones fueron realizadas en vida o como un adorno al embalsamar
las momias.
En América también se han encontrado incrustaciones de oro y piedras
preciosas en dientes de aborígenes pre incas e incas.En Estados Unidos
la Odontología nació en el siglo XVII con la llegada de varios barberos
dentistas de Inglaterra, que fundamentalmente realizaban extracciones.
Estos primeros dentistas habían aprendido su oficio de otros más
experimentados, pero hasta ese momento la Odontología se realizaba sin
ninguna base científica es decir era una odontología empírica.
Investigadores y pensadores con visión de futuro hicieron grandes
avances en la profesión, con PierFauchard la Odontología salió del
empirismo en el siglo XVIII. Fauchard publicó en 1746 la segunda edición
de un libro que contenía los conocimientos odontológicos de la época,
siendo el primero en aconsejar la eliminación de los tejidos cariados y ya
hablaba de un aparato para taladrar dientes.
Fauchard investigó con gran detalle la patología oral, citando numerosas
historias clínicas y el tratamiento que había recomendado. Los problemas
de la dentición le interesaron de gran manera y subrayó la importancia de
retener los dientes caducos hasta que llega el momento en que se caen.
Habló de la reimplantación de dientes arrancados y del trasplante de
dientes de un individuo a otro, anticipándose en cerca de cuarenta años al

trabajo de John Junter. También utilizó la técnica del empaste de una
muela con su posterior relleno de la cavidad con plomo y zinc. Dedicó
especial atención a la odontología protésica y describió cómo construir
puentes individuales además de prótesis parciales y completas. Aconsejó
usar para las prótesis tanto dientes humanos como dientes tallados de
marfil y diseñó métodos para retener en su sitio dentaduras superiores e
inferiores, uniéndolas mediante láminas de acero o muelles en espiral.
Fauchard también construyó tres prótesis sin muelles sujetadas por la
presión atmosférica, pero falló al no apreciar en todo su valor este
principio de retención que mantiene las prótesis en su sitio durante su
uso. No continuó con el estudio de este fenómeno, lo que habría sido un
paso monumental hacia delante. Sin embargo, su trabajo como pionero
en colorear y esmaltar las bases de las dentaduras para simular las
encías naturales inspiró a los que le siguieron en la confección de prótesis
postizas que parecieran más reales y fueran cómodas de llevar.
La comprensión de Fauchard respecto a las enfermedades periodentales
fue muy avanzada para su tiempo. Creía que para prevenir las
enfermedades gingivales era conveniente el raspado de los dientes y el
desbridamiento de las raíces. Partidario de la odontología preventiva,
recomendaba el uso de enjuagues bucales como parte del cuidado diario.
Ilustró su libro con excelentes dibujos de los instrumentos que diseñó, las
prótesis fabricadas y de sus aparatos protésicos. Además, dotó al
gabinete del dentista de una nueva dignidad y lo decoró, insistiendo en
que el paciente, en vez de sentarse en el suelo con el dentista de pie
junto a él, se sentase en un sillón que fuese sólido y firme, adecuado y
confortable, cuyo respaldo debía de ser de pelo de caballo, o con una
almohada blanda más o menos levantada, según la estatura del paciente.
Fauchard se hizo acreedor de gran fama y respeto durante su vida.
Separó con eficacia la odontología del campo de la cirugía y, sobre todo,

del oficio de los sacamuelas, equiparándola a una profesión
independiente, con su campo debidamente circunscrito de deberes y
servicios y su propio nombre. Fue Fauchard quien acuñó él término de
cirujano dentista y cambió el rumbo de la odontología.
En el siglo XIX, (1840) Chapin Harris fundó la primera escuela dental en
Baltimore, este acontecimiento, marcó el nacimiento oficial de la
educación Odontológica.En 1867, la Universidad de Harvard estableció el
primer programa odontológico de carácter universitario.A principios del
siglo XX aparece G. V. Black, quien es el verdadero creador y propulsor
de la Operatoria Dental científica, sus obras editadas, los trabajos
publicados y su infatigable labor docente, sus principios y leyes sobre
preparación de cavidades.
El propósito del presente trabajo es, describir las restauraciones de clase
IV, con el fin de devolver a la pieza dentaria su morfología al mismo
tiempo su función masticatoria y devolviéndole una buena estética al

in coloring and glazing denture bases to simulate
