Currently, the occurrence of accidents and incidents reach high rates in Brazil as fatalities they cause suffering to families and damage to employers. In the case of this study, conducted in a company's cargo transportation business, they were mapped and quantified occurrences of five units in 2014 and held recidivism analysis, assessing the causes and actions taken to identify similar occurrences which could be prevented. Data were recorded by means of graphical representations with rates as number and percentage of occurrence per unit, reason, type, location, root cause and action taken by unit and the company as a whole. Then we compared the occurrence of data to the national percentage of economic activities involved, it is contacting that this falls within the national proportions of accidents with and without registered CAT. With the mapping along the recurrence analysis were identified 20 instances, the main problem is related to the root cause "inadequate environments" in "country roads", generating mostly "material injury". Among these events it was shown that 50% were repeat offenders, so initially the actions taken from the first occurrence had effectively could have avoided 10 events. Therefore, it is proposed as mitigation measure to assess the effectiveness of each action taken, not only briefly in order to eliminate the focus of occurrence but mitigating other variations that may be caused by the same problem. Risk assessment It is also recommended to be revised whenever there are changes in the scenario, each new occurrence or completion of taken actions that influence the severity or frequency of occurrence. A correct analysis of the root cause, with actions focused on the problem and reduce costs, makes the most efficient security management system.
