Seguimiento farmacoterapéutico en pacientes adscritos a un hospital nacional nivel III-E, agosto - noviembre 2019

En el presente estudio, se caracterizó el Seguimiento Farmacoterapéutico en pacientes
adscritos a un hospital nacional de nivel III-E, agosto – noviembre 2019. Para lo cual,
bajo la tutoría de Químicos Farmacéuticos, se realizó el Servicio de Seguimiento
Farmacéutico (SFT) a los pacientes según el servicio hospitalario y bajo la metodología
Dáder, los cuales fueron registrados en las Fichas de SFT y del cual se extrajo toda la
información para la obtención de los resultados. De ello se encontró un total de 314
Resultados Negativos asociados a la Medicación (RNM), siendo el género masculino
(52,54%) el principal género afectado. Por otro lado, la principal clase de RNMque se
identificó fue la inseguridad no cuantitativa (61,15%), mientras que el principal
Problema Relacionado con los Medicamentos (PRM) detectados fueron las interacciones
farmacológicas (68,15%). Así mismo, el 58,6% de los RNM fueron potenciales, por lo
cual es importante que el farmacéutico evite que estos se manifiesten. Los principales
grupos farmacológicos vinculados a RNM fueron los antibacterianos (13,85%) y
antihipertensivos (13,47%). Las principales intervenciones farmacéuticas fueron vigilar
parámetros bioquímicos y de laboratorio (22,61%), seguido retirar un medicamento
(15,92%) y de añadir un medicamento (14,65%). Por último, de todas intervenciones
farmacéuticas realizadas; el 78,86% fueron aceptadas por el profesional médico. Por lo
tanto, el químico farmacéutico tiene un importante papel cuando se integra al Equipo
Multidisciplinario de Salud y participa activamente durante las visitas médicas
interrelacionándose con los demás profesionales de la Salud In the present study, the Pharmacotherapeutic Follow-up was characterized in patients
assigned to a national hospital of level III-E, august - november 2019. For which, under
the guidance of Pharmaceutical Chemists, the Pharmaceutical Follow-up Service (SFT)
was carried out at the patients according to the hospital service and under the Dader
methodology, which were recorded in the SFT files and from which all the information
was extracted to obtain the results. Of this, a total of 314 Negative Outcomesassociated
with Medication (NOM) were found, being the male gender (52.54%) the mainaffected
gender. On the other hand, the main class of NOM that was identified was non-
quantitative insecurity (61.15%), while the main Drug-Related Problem (DRP) detected
was drug interactions (68.15%). Likewise, 58.6% of the NOM were potential, so it is
important for the pharmacist to prevent these from manifesting. The main
pharmacological groups linked to NOM were antibacterial (13.85%) and antihypertensive
(13.47%). The main pharmaceutical interventions were to monitor biochemical and
laboratory parameters (22.61%), followed by withdrawing a drug (15.92%) and adding a
drug (14.65%). Finally, of all the pharmaceutical interventions carried out; 78.86% were
accepted by the medical professional. Therefore, the Pharmaceutical Chemist has an
important role when he is integrated into the Multidisciplinary Health Team and actively
participates during medical visits, interacting with other Health professionals.
