Introduction: Class II malocclusion is one of the most common skeletal alterations in our
population, being the majority of cases originated by mandibular retrognathism. The airway in
this malocclusion is related to a lower average airway volume and this in turn with obstructive
sleep apnea. The use of mandibular propulsors such as the Herbst and Advansync have shown
an increase in airway volume, so they are therapies for the treatment of class II and obstructive
sleep apnea by increasing airway volume.
General objetive: To compare the airway volume in tomographies of patients before and
after Herbst and Advansync therapy in the Postgraduate Orthodontic Clinic of the Faculty of
Dentistry of the University of Cuenca in the period May 2017-January 2018.
Methodology: For this retrospective longitudinal study, CBCT scans of 29 subjects aged 11
to 17 years were used, 15 subjects were treated with the Herbst appliance and 14 with the
Advansync appliance. The tomographies were evaluated using NemoFAB software, Nemostudio
2020 before and after using the mandibular advancement device.
Results: In the comparison of T1 and T2 measurements in nasopharynx, oropharynx and
total volume, a statistically significant difference was determined due to the increase in the
volume of the segments. On the other hand, the hypopharynx did not show a statistically
significant difference when comparing air volumes in T2 with T1.
Discussion: DAMs have become one of the main options for the treatment of respiratory
disorders due to their effectiveness in increasing airway volume, however, it is recommended
to evaluate their long-term efficacy.
Conclusions: DAMs increase total pharyngeal, nasopharyngeal and oropharyngeal volume,
while in the hypopharynx there is no statistically significant difference. Airway volume variations
are not related to the sex of the patients. There is no statistically significant difference when
comparing the increase in airway volume in patients who used the Herbst or Advansync
appliance
