Objective: The aim of this integrative review was to address the causes and clinical management of the canal with persistent exudate or “weeping canal”, which is a clinical entity characterized by the presence of persistent inflammatory exudate in the canal, despite ongoing root canal treatment, even after multiple appointments. Methodology: This article has a qualitative methodological approach. A bibliographic survey was carried out in the Pubmed, Scopus and EBSCO databases, in English and without restriction of publication date. The search terms used in combination were: wet root canal, weeping canal, persistent exudate, persistent intracanal exudate, and root canal treatment. Results and conclusion: With this article it is concluded that the main cause described is microbial, in the form of a bacterial biofilm or recalcitrant infection, without neglecting the chemical cause and the mechanical cause. Regarding its clinical management, a series of approaches are available, such as paper points, intra-canal dressing, such as calcium hydroxide and triple antibiotic paste, the use of aspiration systems such as EndoVac, while the use of Nd: YAG laser therapy requires more studies to support its clinical use and ultimately endodontic surgery. It should be taken into consideration that the use of these approaches can be performed independently or using two or more approaches in combination for the clinical management of the canal with persistent exudate.
