Impacto de la profilaxis con palivizumab en la hospitalización de recién nacidos de alto riesgo para infección por virus sincitial respiratorio en el Hospital Militar Central

Objetivo
Conocer las características de los pacientes de alto riesgo, que son hospitalizados con infección respiratoria baja en el Hospital Militar Central, luego de recibir o no profilaxis con Palivizumab.
Métodos
Se realizó un estudio observacional descriptivo prospectivo, en el que se incluyeron lactantes que consultaron al servicio de urgencias del Hospital con sintomatología respiratoria baja. Se realizó una muestra de secreción nasofaríngea para evaluar la presencia o no por virus sincitial respiratorio (VSR). Se indagó sobre la exposición a palivizumab, necesidad de hospitalización y/o requerimiento de UCI.
Resultados
Se incluyeron en el análisis 240 pacientes con un promedio de edad de 2.7 meses y un 63.4 % de población masculina. Del total de los pacientes, 21.7% eran prematuros, el principal diagnóstico fue bronquiolitis en un 63.5% y neumonía viral en un 23.1%. Un 75% de los pacientes prematuros tenían el antecedente de hospitalización al nacer, en comparación con un 13.8% de los pacientes no prematuros (p<0.001), los pacientes prematuros requirieron mayor soporte ventilatorio y oxígeno. Al analizar el uso de palivizumab en aquellos pacientes con indicación de inmunoprofilaxis, el 71.4% lo recibieron con un promedio de 2.5 dosis. La prueba de VSR fue negativa en 11 pacientes (78.5%); de 3 pacientes con resultado positivo, uno necesitó hospitalización en UCI con soporte ventilatorio. Se observó un mayor promedio de días de hospitalización en aquellos pacientes que recibieron más de 3 dosis de Palivizumab. La prevalencia de hospitalización durante el año fue mayor durante los meses de marzo a junio.
Conclusiones
Se observó que la tasa de hospitalización en los pacientes considerados de alto riesgo en nuestro hospital es elevada, siendo la principal causa el requerimiento de oxígeno suplementario. Una vez hospitalizados se observa que los días de hospitalización son más elevados en aquellos pacientes que recibieron Palivizumab, probablemente por ser aquellos con un peor pronóstico. Esta hospitalización se incrementa durante los picos respiratorios de VSR durante los meses de marzo a junio.
