Acute pancreatitis is a pathology with a high incidence where identifying the severity to establish a safe treatment is perhaps one of the most important aspects. Different scales have been developed that objectively allow risk stratification in pancreatitis. It is proposed to determine the concordance of the risk of severity estimated by the BISAP scale and those estimated by Marshall and APACHE in patients with pancreatitis with a concordance study between the BISAP, Marshall and Apache scales, for the measurement of the severity of pancreatitis. A total of 424 were included in the analysis, 61% were severe, 15% moderately severe, and 24% mild. Among the severe cases, 87% were classified as such due to respiratory failure, of which the mean PaFi was 275. The Kappa value for the relationship between the severity of APACHE II and BISAP was 0.19 (slight strength of agreement). What than for the relationship between the severity of Marshall and BISAP of 0.12. We conclude that the severity of acute pancreatitis in our population is overestimated, possibly secondary to the fact that this severity is calculated with scales that include the PaFi value (Marshall and APACHE II).
