dc.description.abstractIntroduction: Colorectal cancer (CRC) is a high incidence neoplastic entity in the Colombian context, with figures that concern and put the health system on alert. This type of cancer involves an experience linked to pain, symptoms, limitation and mutilation; modification of lifestyles due to the characteristics of the treatments; fear and threat of death; and socialization disorders, with repercussions for both the patient and the family. Objective: To determine the efficacy of a nursing intervention for self-management and care competence in adults with colorectal cancer undergoing surgical treatment. Method: Quantitative, quasi-experimental, longitudinal-section approach study that included 104 patients diagnosed with RCC who underwent surgical intervention with curative intent. They were randomly distributed in two groups, the experimental group (GE) made up of 52 patients who received the PEACCR nursing intervention (Colorectal Cancer Self-Management Training Program) from admission to the health institution and the control group were divided into two groups. (CG) in which 52 patients participated, who were followed up with the conventional intervention. The PEACCR intervention was carried out in 6 weeks and consisted of 3 face-to-face sessions on knowledge, beliefs, alliance with the health team, coping and management of symptoms and ostomies, 3 telephone sessions. The study was endorsed by the Institutional Research and Ethics Committees. Results: Profile of the GC and GE participants, at the beginning of the intervention: Most are female, with age ranges from 32 to 87 years, average of 56.7 years, of contributory or subsidized regime, educational level primary baccalaureate, residents in the urban area of Bogotá. Regarding medical diagnosis, adenocarcinoma of the rectum or colon predominates, most of which required hemicolectomy with anastomosis, and required some type of colostomy. Prior to the intervention, both the CG and GE participants presented average levels of self-management behaviors and competence for home care. In the GE, at month 1 of the PEACCR intervention, an increase in self-management behaviors was identified. In month 2, a significant increase in these behaviors is evident and in all four dimensions with statistically significant changes. In the CG, a month after the conventional intervention, a slight increase in the dimensions of alliance / health personnel (average range 40.02, p value 0.000) and knowledge (average range 51.41, p value 0.712) is identified without these changes are significant. At month 2, the values of self-management behaviors (average range 40.98, p value 0.000), and the dimensions of the personal health alliance (average range 40.11, p value 0.000) increased with statistically significant changes. Regarding the competence for home care, prior to the intervention, both the GE and the CG present high levels. In the EG, these levels are maintained at month 1 of the intervention, and in month 2 they are further increased, with statistically significant changes. In the CG, likewise, these levels are maintained at month 1 of the conventional intervention, and in month 2 they are increased, with statistically significant changes only in the dimensions of competence for care, knowledge and enjoyment. The PEACCR intervention increases self-management behaviors and competence for home care in adults with colorectal cancer. It is consolidated as an intervention to be taken into account in the formulation of cancer monitoring plans and programs in accordance with care models focused on the needs of persons and their families.
dc.description.abstractIntroducción: El cáncer colorrectal (CCR) es una entidad neoplásica de alta incidencia en el contexto colombiano, con cifras que preocupan y ponen en alerta al sistema de salud. Este tipo de cáncer implica una experiencia ligada a dolor, síntomas, limitación y mutilación; modificación de estilos de vida por las características de los tratamientos; temor y amenaza de muerte; y alteraciones de socialización, con repercusiones tanto para el enfermo como para la familia. Objetivo: Determinar la eficacia de una intervención de enfermería para el automanejo y la competencia de cuidado en adultos con cáncer colorrectal en tratamiento quirúrgico. Método: Estudio de abordaje cuantitativo tipo cuasiexperimental de corte longitudinal que incluyo 104 pacientes con diagnóstico de CCR sometidos a intervención quirúrgica con intención curativa. Se distribuyeron aleatoriamente en dos grupos, así, el grupo experimental (GE) conformado por 52 pacientes quienes recibieron la intervención de enfermería PEACCR (Programa para el Entrenamiento en Automanejo en Adultos con Cáncer Colorrectal) a cada uno de los participantes desde el ingreso a la institución de salud y el grupo control (GC) en el que participaron 52 pacientes a quienes se realizó seguimiento de la intervención convencional. La intervención PEACCR se adelantó en 6 semanas y consistió en 3 sesiones presenciales sobre conocimiento, creencias, alianza con el equipo de salud, afrontamiento y manejo de síntomas y ostomías, 3 sesiones telefónicas. El estudio contó con aval de los Comités de ética e investigación institucionales. Resultados: Perfil de los participantes del GC y GE, al inicio de la intervención: La mayor parte son del género femenino, con rangos de edad de 32 a 87 años, promedio de 56,7 años, de régimen contributivo o subsidiado, nivel educativo bachillero primaria, residentes en el casco urbano de Bogotá. En cuanto al diagnóstico médico, predomina el adenocarcinoma de recto o colon, la mayor parte requirió hemicolectomía con anastomosis, y requirió algún tipo de colostomía. Previo a la intervención los participantes tanto del GC y GE, presentan niveles medios de comportamientos de automanejo y de competencia para el cuidado en el hogar. En el GE, al mes 1 de la intervención PEACCR se identifica un aumento de los comportamientos de automanejo. En el mes 2 se evidencia un aumento significativo de estos comportamientos y en sus cuatro dimensiones con cambios estadísticamente significativos. En el GC, al mes de la intervención convencional se identifica un leve aumento en las dimensiones alianza/personal de salud (rango promedio 40,02, p valor 0,000) y conocimientos (rango promedio 51,41, p valor 0,712) sin que estos cambios sean significativos. Al mes 2 aumentaron los valores de comportamientos de automanejo (rango promedio 40,98, p valor 0,000), y las dimensiones alianza personal de salud (rango promedio 40,11, p valor 0,000) con cambios estadísticamente significativos. Respecto a la competencia para el cuidado en el hogar, previo a la intervención tanto el GE como el GC presentan niveles altos. En el GE, estos niveles se mantienen al mes 1 de la intervención, y en el mes 2 se aumentan aún más, con cambios estadísticamente significativos. En el GC, de igual forma, estos niveles se mantienen al mes 1 de la intervención convencional, y en el mes 2 se aumentan, con cambios estadísticamente significativos únicamente en las dimensiones de competencia para el cuidado, conocimiento y disfrutar. La intervención PEACCR permite aumentar los comportamientos de automanejo y la competencia para el cuidado en el hogar en adultos con cáncer colorrectal. Se consolida como una intervención a ser tenida en cuenta en la formulación de planes y programas de seguimiento en cáncer acorde con los modelos de cuidado centrados en las necesidades de las personas y sus familias.
