dc.description.abstractObjetivo: Identificar criterios de deprescripción de medicamentos en pacientes adultos mayores con diabetes mellitus tipo 2. Métodos: Esta tesis doctoral se realizó en cuatro etapas. La primera se condujo mediante una revisión sistemática de la literatura para identificar herramientas útiles en el proceso de deprescripción. La segunda se centró en la realización de un estudio descriptivo de una cohorte de más de 30.000 adultos mayores colombianos con diabetes mellitus tipo 2, a los cuales se les aplicaron los criterios de Beers. La tercera consistió en la valoración de la validez facial y de contenido de un algoritmo y un listado de criterios para la deprescripción de medicamentos, utilizando la razón y el índice de validez de contenido y el kappa de Fleiss. La cuarta se orientó a establecer una aproximación a la potencial eficiencia de la deprescripción para el sistema de salud a través de un análisis de minimización de costos y de impacto presupuestal de la insulina glargina comparada con la NPH. Resultados: Se identificaron 63 herramientas, 50 criterios explícitos y 13 algoritmos de desprescripción. El uso de los criterios de Beers en la cohorte de pacientes mostró que la prescripción potencialmente inapropiada más frecuente se asoció con los inhibidores de la bomba de protones (23.39%), a la vez que permitió evidenciar limitaciones en la aplicación a innovaciones terapéuticas. El algoritmo validado por 18 expertos internacionales es un aporte al estado del arte, a la par que incorpora tanto preguntas que atienden a la práctica habitual y como una lista de recomendaciones de deprescripción para 33 grupos farmacoterapéuticos. El análisis de minimización de costos de la deprescripción de un solo medicamento mostró ahorros y una mayor eficiencia de recursos del sistema de salud. Conclusión: el algoritmo y criterios propuestos son herramientas ajustadas al contexto colombiano que pueden ser útiles para apoyar la deprescripción en adultos mayores.
dc.description.abstractObjective: To identify criteria for deprescribing medication in elderly patients with type 2 diabetes mellitus. Methods: This thesis was carried out in four phases. The first phase consisted on a systematic review of the literature in order to identify tools for deprescribing. In the second phase, a descriptive study of a cohort of more than 30.000 elderly Colombian patients with type 2 diabetes mellitus to which the AGS Beers v.2019 criteria were applied, was conducted. The third phase consisted on establishing facial and content validity for an algorithm and listing criteria for deprescribing medications in elderly patients, using the content validity ratio, the content validity index and the Fleiss’ Kappa. In the fourth phase, a calculation of the potential efficiency gains for the healthcare system of the proposed deprescribing process were carried out, following a cost minimization analysis of insulin glargine compared to NPH insulin and a budget impact analysis. Results: A total of 63 tools were identified; 50 referred to explicit criteria and 13 to algorithms for deprescribing. Beers criteria were applied to a 30.000 patient cohort and it showed that the most frequent potentially inappropriate prescription was the use of proton pump inhibitors (23.39%); it allowed to show limitations in the application to therapeutic innovations. The algorithm validated by 18 international experts is a contribution to the state-of-the-art and incorporate questions that address the process in the real clinical practice. The explicit list included recommendations for 33 criteria. The cost minimization analysis of the deprescription of a single drug showed savings and greater efficiency in the use of health system resources. Conclusion: the proposed algorithm and criteria list are tools adjusted to the Colombian context that can be useful to support deprescription in elderly patients.
