E: To determine if the Polymorphism G-2548A of the LEP gene, A223G of the LEPR gene and C34G of the PPARɣ gene, are genetic factors of risk for an unfavorable outcome in secondary peritonitis. MATERIAL AND METHODS: Prospective cohort. 75 patients of the service of general surgery of the General Hospital of Durango, SSA were included, with diagnose of peritonitis secondary non apendicular, confirmed surgically. Were excluded patient with peritonitis apendicular, pancreatic peritonitis, immonosuppressive treatment or chronic corticosteroids intake. Was done extraction of DNA starting from total blood, PCR final point, and mutation specifies, as well as RFLP. RESULTS. 75 patients were includ (42♂ and 33♀), the age average of men survivors was of 43 and 55 year-old of women surviors. A Body Mass Index (BMI) average a total patients (24.20♀ and 23.6♂). Of the 75 patients, 22 died, and 53 survived. Analysis of risks. The mutated allele (A) to polymorphism of the LEP gene represents 4.64 times more non survival risk individuals with wild allele (G). With regard to the polymorphism A223G of the gene LEPR, the presence of the allele muted (G), suggests the possibility of a factor protective 3.5 times, however the interval of trust is not definitive. The polymorphism C34G of the gene PPARɣ doesn't represent a factor of risk for unfavorable outcome
