Caso raro de lipoma gigante en mano de niño : reporte de caso

Abstract

The case of an 8-year-old child is described in this paper. He was brought to our consult because of a mass in his left hand present for 6 months with progressive growth. It was located on the third finger with deep extension at the level of the interdigital commissure between the second and third fingers. It became noticeable, deforming the morphology of the hand and interfering with school work both psychologically and functionally while affecting the grip and pinch. An x-ray was brought which showed a radiolucent mass of soft tissue and a magnetic resonance imaging (MRI) was requested to define surgical behavior. It identified a mass of macrolobulated lipomatous origin of 62x24x27 mm, without bone, ligament or tendon involvement. Between the MRI and the surgical programming including time delay in administrative procedures, it takes another 10 months to perform the surgery. Due to size and extension of the tumor, both dorsal and anterior approaches are required at surgery. The anatomopathological diagnosis of the mass is reported as lipoma. Postoperative evolution was satisfactory, without any deficit and without complications. The realization of this case report was considered important because, as far as the researchers know, no report of giant lipoma of the hand was found in a paediatric population and its repercussions on the decision-making process are relevant when they are weighted against other similar pathologies. In additon, a literature review on soft tissue tumors in the hand and more specifically of giant lipomas and their rare presentation in the pediatric population was conducted
