Home care is now a care approach that allows to solve the difficulties arising from the hospital, chronic overcrowding, which is a problem of public health concern in developed countries and can be managed in the patient's home as an option cost-effective and safe. For this it is necessary to find strategies for its development, and risk management models of care, thus improving the health conditions of the population.
One of the main challenges of managing health care programs, is to define the areas in which to intervene to improve efficiency and quality in service delivery, so that these aspects are constituted as determinants of patient care and his family. This paper identifies the key determinants in the care of people with aftermath of stroke who receive home health management, in order to identify priority areas of intervention, ensuring improved clinical management in three specific areas: caregiver burden, polypharmacy and pressure ulcers.
