Orientador: Luciana Asprino Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: A osteotomia sagital do ramo mandibular (OSRM) é um procedimento cirúrgico amplamente utilizado para a correção de deformidades dentofaciais, tanto para avanços quanto para recuos mandibulares. Uma das possíveis complicações dessa técnica é o dano ao nervo alveolar inferior. Sendo assim, o objetivo no presente estudo foi avaliar a posição do forame e canal mandibulares em indivíduos de diferentes classes esqueléticas. Para isso, foram utilizadas 90 imagens de Telerradiografia em Norma Lateral e 90 imagens de Tomografia Computadorizada de Feixe Cônico, que pertenciam aos mesmos indivíduos, sendo 50 do sexo feminino e 40 do sexo masculino, com idade entre 18 e 30 anos. As imagens foram selecionadas a partir de um arquivo de exames de pacientes atendidos na Clínica de Radiologia da FOP ¿ Unicamp e foram avaliadas por um radiologista com experiência na análise das imagens utilizadas no estudo. Os indivíduos foram classificados em diferentes tipos esqueléticos (Classe I, Classe II e Classe III) a partir da análise cefalométrica de Steiner, realizada na telerradiografia. Após a determinação dos tipos esqueléticos, foram realizadas mensurações lineares nas tomografias computadorizadas de feixe cônico, relacionadas à posição do forame e do canal mandibulares, utilizando o software Dolphin 3D® (Dolphin Imaging, Chatsworth, CA, USA). Foram mensuradas as distâncias do forame mandibular até a porção lingual da região anterior do ramo da mandíbula, do forame mandibular até a incisura da mandíbula, do forame mandibular até o plano oclusal e a espessura do ramo mandibular acima do forame Foram mensuradas, também, a distância do canal mandibular até a crista alveolar, do canal mandibular até a base da mandíbula e do canal mandibular até a cortical vestibular da mandíbula, além da espessura do corpo da mandíbula, a largura e a altura do canal mandibular. Para comparação entre as classes esqueléticas e as mensurações realizadas, foram utilizados os testes estatísticos two-way ANOVA e Tukey, com nível de significância alfa de 5%. A fim de analisar possíveis correlações entre as mensurações, foi realizado o teste de Pearson-rP. Os resultados mostraram que as mensurações realizadas não apresentaram diferenças estatisticamente significantes entre as classes esqueléticas, exceto para a posição do forame mandibular em relação à incisura da mandíbula entre as classes II e III para o lado direito. Não foram observadas correlações exclusivas de uma só classe, indicando que a classe não influencia diretamente na relação entre as medidas. Pode-se concluir que o forame mandibular apresentou-se localizado mais superiormente, ou seja, mais próximo à incisura da mandíbula, na classe III do que na classe II. Não houve diferença na posição do forame mandibular no sentido anteroposterior entre as classes esqueléticas e entre os lados. Não se observou diferença na posição do canal mandibular entre as classes esqueléticas. O presente estudo sugere que não há necessidade de cuidados técnicos diferenciados na execução da OSRM para cada classe esquelética, e sim individualizados para cada paciente Abstract: The sagittal split mandibular osteotomy is a surgical procedure widely used for correction of dentofacial deformities in mandibular advances and setbacks. One of the possible complications of this technique is the damage to the inferior alveolar nerve. Thus, the aim of this study was to evaluate the position of the foramen and mandibular canal in individuals of differents skeletal pattern. This study examined 90 images (50 females and 40 males) of cone beam computed tomography and lateral cephalometric radiographs of the same patient. They were aged between 18 and 30 years and have been indicated to the Dental Radiology Clinic of Piracicaba Dental School - UNICAMP with different indications. All the images were evaluated by a radiologist with experience in the analysis of these types of exams. The lateral cephalometric radiographs images were classified in differents skeletal patterns (Class I, Class II and Class III) using Steiner's cephalometric analysis. After determination of the skeletal types, linear measurements were made on cone beam computed tomography, to find the position of the foramen and mandibular canal, using the software Dolphin 3D® (Dolphin Imaging, Chatsworth, CA, USA). It was measured the distances: from the mandibular foramen to the lingual portion of the anterior region of mandible¿s ramus; from de mandibular foramen to the notch of the mandible; from the mandibular foramen to the occlusal plane; from mandibular canal to alveolar crest; from the mandibular canal to mandible¿s boarder; from the mandibular canal to the buccal cortex. The thickness of the mandibular ramus above the foramen, the thickness of mandible¿s body and the width and height of the mandibular canal were also measured. It was evaluated the relationship between skeletal classes and all the measurements, by analysis of statistical tests (two-way ANOVA and Tukey) with significance level of 5%. In order to analyze the correlation between the measurements it was performed the Pearson RP-test. Results showed that measurements performed had no statistically significant differences between skeletal patterns, except for position of mandibular foramen in relation to mandibular notch between classes II and III to the right side. It was not observed unique correlations, indicating that skeletal pattern did not directly influenced the relationship between measures. It can be concluded that the mandibular foramen is superiorly located in class III than in classe II. There was no difference in the position of the mandibular foramen in the anteroposterior direction between skeletal patterns and between the sides. There was no difference in the position of mandibular canal between skeletal patterns. This study suggests that there is no need for different technical care in the execution of OSRM for each skeletal pattern, but individualized for each patient Mestrado Radiologia Odontologica Mestra em Radiologia Odontológica
