dc.description.abstractEn la actualidad, hay evidencia de que la Enfermedad Renal Crónica (ERC) aumenta de forma significativa día a día, generando una carga de enfermedad terminal importante, convirtiéndose así en un problema de salud pública. Por ser esta más prevalente en la edad adulta, se acostumbra abordarla con el enfoque propio de la adultez, en la medida en que la obesidad, la diabetes y la hipertensión son las causas más frecuentes de ERC que requieren un manejo nutricional desde su inicio. Por su parte, en los niños, la enfermedad renal aguda y crónica requiere también un soporte nutricional desde etapas muy tempranas de la vida. Las anormalidades congénitas de los riñones y las vías urinarias (CAKUT, sigla en inglés), son las causantes del 50-55 % de la enfermedad renal crónica en niños y adolescentes. Adicionalmente, se suma ese 10 a15 % que generan los niños prematuros y/o con Retardo del Crecimiento Intra Uterino (RCIU) que, al final de su desarrollo postnatal, quedan con una masa renal disminuida, colocándolos en riesgo de deterioro de la función renal en etapas tempranas de la vida. Existe otro grupo de pacientes causante de ERC, que son los niños que desde la infancia exhiben enfermedades glomerulares renales como el síndrome nefrótico, con pérdidas sustanciales de proteínas y micronutrientes por orina u otros que presentan tubulopatías, en donde las pérdidas de electrolitos, aminoácidos o sustancias buffers por orina generan un impacto negativo sobre el desarrollo pondoestatural y sicomotor del niño. Este grupo poblacional es el causante del 15-20 % de ERC. Todas estas enfermedades se vuelven aún más importante cuando se encuentran en períodos especiales de la vida como es el crecimiento rápido en los primeros tres años de vida, la adolescencia, embarazo o procedimientos quirúrgicos mayores, convirtiéndose en poblaciones que requieren una intervención o asesoría nutricional adecuada como pilar fundamental para su manejo. Estas causas de ERC en el niño, que desde su inicio tienen el acompañamiento de los pediatras, urólogos, cirujanos pediátras y nefrólogos pediatras, requieren un abordaje especial, el cual no es suficientemente cubierto por las anteriores especialidades. Razón por la que el apoyo del área de la Nutrición se convierte en un soporte fundamental en el manejo de estos pacientes. La nutrición y alimentación especializada y su planeación deben estar enfocados desde los riesgos inherentes a cada patología renal y al acompañamiento durante el tiempo de presentación de la enfermedad. Fue esta consideración y la escasez de textos a nivel nacional e internacional acerca de la nutrición en niños con enfermedades renal la que nos hizo conscientes de la imperiosa necesidad de apoyar el manejo médico, y de hacer un libro que sirva de ayuda a los clínicos para diseñar unas pautas o guías de manejo acerca de la Nutrición para este grupo de pacientes y sus familias. Por ello, convocamos a expertos Pediatras, Nefrólogos Pediatras, Nutricionistas Fisiólogos, Bioquímicos, Neonatólogos, Enfermeras y Médicos que participan en la atención directa de nuestros pacientes, a sintetizar las recomendaciones nutricionales para cada patología renal de acuerdo a las experiencias, evidencias y recomendaciones más actualizadas. La convocatoria fue muy bien recibida por los autores de cada capítulo, colocando cada uno de ellos todo su entusiasmo en la escritura inicial, así como en la corrección de los textos cuando esto fue necesario. A los participantes regionales y nacionales: Natalia Mejía, Consuelo Restrepo de Rovetto, Margarita Calle y Jorge Endo (nefrólogos pediatras), Isabella Echeverry (fisióloga), Adriana Ballesteros (neonatóloga), Armando Lucumí (bioquímico), Rosales Hooker, Valeria Restrepo y María I Hooker (médicos), Mabel Libreros, Yara Oñate y Julio Cesar Castro (pediatras), Luis Becerra y Lucy Cuellar (nutricionistas nacionales), Nelly Johana Ramirez, Pilar Milkey, Anais Camacho, Carolina, Avalos y Julieta Gaggino (nutricionistas internacionales de Chile, México y Argentina, respectivamente). A todos ellos queremos agradecerles su generosidad y paciencia ante las solicitudes hechas para sacar adelante los diferentes capítulos en tiempos difíciles como fue el dedicado durante la pandemia del COVID-19. Un agradecimiento muy especial a las doctoras Vanessa Ochoa y Jessica Forero, que fungieron como editoras invitadas desde su segundo año del entrenamiento en Nefrología Pediátrica del programa Fundación Valle del Lili-Universidad Icesi hasta un año posterior a su graduación, ya como nefrólogas pediatras. Esperando que este texto logre el objetivo principal para el cual fue concebido: ser una herramienta para informar, enseñar y orientar tanto a los médicos generales, pediatras, nefrólogos pediatras, nutricionistas, estudiantes de pregrado y docentes de pediatría como de nutrición en las alteraciones renales. Si este objetivo se cumple, creemos haber dado un paso importante en el tratamiento integral y en la disminución de la morbimortalidad en este grupo de pacientes.
