| dc.description.abstract | Objetivo: Demostrar la asociación entre hipoalbuminemia (albúmina <3,5 g/dl) y la
morbilidad postoperatoria en cáncer gástrico.
Material y métodos: Se llevó a cabo un estudio analítico, longitudinal retrospectivo
de y casos-controles anidado en una cohorte. Se incluyeron 150 pacientes
gastrectomizados por cáncer gástrico en el Instituto Regional de Enfermedades
Neoplásicas “DR. Luis Pinillos Ganoza” – IREN NORTE, en el periodo 2011-2023,
entre ellos, 50 pacientes gastrectomizados por cáncer gástrico si desarrollaron
complicaciones postoperatorias y 100 pacientes gastrectomizados por cáncer
gástrico no desarrollaron complicaciones post operatorias.
Resultados: los pacientes con hipoalbuminemia, un 42,6% mostró complicaciones
postoperatorias, en cuanto a las complicaciones intraabdominales, se encontró,
dehiscencia y fistula de anastomosis (4,3%) e ISO (12,8%). Asimismo, con respecto
a las complicaciones extraabdominales, se evidenció, atelectasia (4,3%), neumonía
(12,8%) y sepsis (12,8%). Cabe destacar que, entre los grupos con y sin
hipoalbuminemia respecto a los factores estudiados, no tuvieron significancia
estadística, excepto en la variable anemia (p<0,05). En el análisis bivariado, el
estadío patológico estuvo significativamente asociado a complicaciones generales
(OR = 2,56; IC 95% = 1,04 – 6,30; p = 0,04) e intraabodminales (OR = 1,14; IC 95%
= 1,02 – 1,26; p = 0,03); la edad >= 63 años fue asociado a complicaciones generales
(OR = 2,36; IC 95% = 1,17 – 4,75; p = 0,02) y extraabdominales (OR = 2,63; IC 95%
= 1,20 – 5,74; p = 0,01). Asimismo, en el análisis multivariado, el estadío patológico
se asoció significativamente a las complicaciones intraabdominales (OR = 15,08; IC
95% = 1,05 – 216,49; p = 0,04).
Conclusión: La hipoalbuminemia no es un factor pronóstico de morbilidad
postoperatoria en pacientes con cáncer gástrico | |
