dc.description.abstract | Identificar los factores de riesgo asociados a la pancreatitis post-CPRE
en aquellos pacientes que acuden para CPRE por coledocolitiasis.
Materiales y método: El presente estudio tiene un diseño de tipo analítico,
observacional de casos y controles. Se incluyó pacientes adultos con diagnóstico
de coledocolitiasis a los cuales se les realizó CPRE en el servicio de
Gastroenterología del Hospital De Alta Complejidad Virgen De La Puerta –
EsSalud Trujillo. Mediante un muestreo aleatorio simple, se seleccionaron un
total de 220 pacientes (44 casos y 176 controles), atendidos durante el periodo
octubre 2021 a abril 2023.
Resultado: Se analizaron factores de riesgo relacionados con el procedimiento
y aquellos relacionados con el paciente. Se determinó que los pacientes con
canulación difícil tienen 5 veces más riesgo de presentar pancreatitis post-CPRE
(OR=5.07, IC95%=2.49 – 10.34, p=0.000). De manera similar, la dilatación del
conducto colédoco >9mm resulta aumentar 2 veces el riesgo de pancreatitis
post-CPRE (OR=2.80; IC95%= 1.18 – 6.66; p=0.0161). Sin embargo, la
extracción de >1 cálculos del conducto colédoco y otros factores asociados a los
pacientes como el género femenino y la edad <60 años no resultaron ser
estadísticamente significativos.
Conclusiones: Los factores de riesgo asociados a la pancreatitis post-CPRE
son la canulación difícil y la dilatación del conducto colédoco >9mm. | |
dc.description.abstract | To identify the risk factors associated with post-ERCP pancreatitis in
those patients who come for ERCP due to choledocholithiasis.
Materials and method: This study has an analytical design, observational
technique, cases and controls. Adult patients with a diagnosis of
choledocholithiasis who underwent ERCP were included in the Gastroenterology
service of the Virgen De La Puerta High Complexity Hospital - EsSalud Trujillo.
Through simple random sampling, a total of 220 patients (44 cases and 176
controls) were selected, attended during the period October 2021 to April 2023.
Result: Risk factors related to the procedure and those related to the patient
were analyzed. It was determined that patients with difficult canulation have a 5
times higher risk of presenting post-ERCP pancreatitis (OR=5.07, 95%CI=2.49 –
10.34, p=0.000). Similarly, dilatation of the common bile duct >9mm is found to
increase 2-fold the risk of post-ERCP pancreatitis (OR=2.80; 95%CI=1.18 – 6.66;
p=0.0161). However, removal of >1 common bile duct stones and other patient associated factors such as female gender and age <60 years were not found to
be statistically significant.
Conclusions: The risk factors associated with post-ERCP pancreatitis are
difficult canulation and dilation of the common bile duct >9mm. | |
