Evaluación del Trastorno Afectivo Bipolar: revisión sistemática de los estudios instrumentales

dc.descriptionBipolar Disorder is a widely studied construct, but its diagnostic process has been controversial, because the use of measuring instruments is a relatively new practice. Objective: To carry out a Systematic Review of the instruments mostly used, to identify in the literature their conceptual adequacy, the method of administration and the psychometric properties of the measurement instruments in Spanish-speaking adults between 18 and 65 years of age. Electronic searches were carried out in: Scopus, Science Direct and Wed of Science, without time of year; using key words based on Methodological and Clinical Terms of the Construct, connected by the Boolean operator “AND”. Methodological quality was assessed using the COSMIN checklist. Result: After preselection and elimination of duplicate studies, 28 suitable studies remained. The most used instruments were: MDQ, HAMD-5, BDI and IDS. 12 studies reported the theoretical model, the DSM-IV and the affective and emotional composite temperament model, being the most widely used. Regarding psychometric properties, 13 instruments determined construct validity through Factor Analysis, the most used extraction method for identifying factor load is exploratory factor analysis, using the adjustment index: component analysis main. 24 studies reported internal consistency analysis in their methodology, using Cronbach's α coefficient. Pearson's correlation was the most frequently performed statistic for evaluating concurrent validity. Finally, in construct validity, 8 studies used correlation analyzes (convergent and discriminant).
dc.descriptionEl Trastorno Bipolar es un constructo ampliamente estudiado, pero su proceso diagnostico ha sido controvertido, porque el uso de los instrumentos de medición es una práctica relativamente nueva. Objetivo: Realizar una Revisión Sistemática de los instrumentos mayormente empleados, para identificar en la literatura su adecuación conceptual, la forma de administración y las propiedades psicométricas de los instrumentos de medición en adultos hispanohablantes de 18 a 65 años. Se realizaron búsquedas electrónicas en: Scopus, Science Direct y Wed of Science, sin temporalidad de año; utilizando palabras claves en función a términos metodológicos y clínicos del constructo, conectados por el operador booleano “AND”. La calidad metodológica se evaluó mediante la lista de verificación COSMIN. Resultado: Tras la preselección y eliminación de estudios duplicados quedaron 28 estudios idóneos. Los instrumentos más empleados fueron: MDQ, HAMD-5, BDI y IDS. 12 estudios reportaron modelo teórico, siendo el DSM-IV y el modelo de temperamento compuesto afectivo y emocional los más empleados. En cuanto a las propiedades psicométricas, 13 instrumentos determinaron la validez de constructo mediante el análisis factorial, el método de extracción más usado para la identificación de la carga de los factores es el análisis factorial exploratorio, utilizando el índice de ajuste: el análisis de componentes principales. 24 estudios reportaron en su metodología análisis de consistencia interna, utilizando el coeficiente α de Cronbach. La correlación de Pearson fue el estadístico más efectuado para la evaluación de la validez concurrente. Finalmente, en la validez de constructo, 8 estudios utilizaron los análisis de correlación (convergente y discriminante).
