dc.description | Este trabalho descreve os efeitos do óxido nítrico em recémnascidos com hipertensão pulmonar persistente. Estudamos 9 recém-nascidos com insuficiência respiratória grave caracterizada por hipoxemia acentuada e hipertensão pulmonar. Em todos os recém-nascidos havia indicação de utilização de oxigenação de membrana com circulação extra-corpórea (ECMO) e o índice de oxigenação (OI) era superior a 25. O peso médio dos pacientes foi de 2648 ± 661g e a idade gestacional 36,4 ± 2,6 semanas. O óxido nítrico foi administrado em circuito em Y na linha inspiratória do circuito do respirador, de um tanque com 1000 ppm. As concentrações de NO e NO2 foram aferidas com monitores eletroquímicos (PACI e PACII-Draeger) na linha expiatória dos pacientes. O diagnóstico de hipertensão pulmonar foi feito com base nos sinais clínicos e ecocardiográficos. A pressão da artéria pulmonar foi estimada através de ecardiografia e o shunt pelo canal arterial e/ou forâmen oval confirmado por doppler colorido. Todos os pacientes mostraram uma melhora significativa na oxigenação arterial após a administração do óxido nítrico. A concentração inicialmente utilizada foi de 20 ppm. Trinta minutos após o início da administração do óxido nítrico, o índice de oxigenação médio, que era 48,5, caiu para 17,7, e com 6 h e 12 h de óxido nítrico, o índice de oxigenação era de 14,1 e 10,5 respectivamente. O óxido nítrico levou a uma redução importante da resistência vascular pulmonar, traduzida por queda significativa na pressão da artéria pulmonar e inversão do shunt direito esquerdo através do canal ou forâmem oval. Não observamos alteração na pressão arterial sistêmica com o uso do óxido nítrico e os níveis plasmáticos de metaemoglobina permaneceram abaixo de 1,5%. Dos 9 pacientes estudados, apenas 1 foi a óbito, após ter revertido o quadro de hipertensão pulmonar, em conseqüência de complicações de asfixia perinatal. | |
dc.description | This study describes the effects of nitric oxide in newborns with persistent pulmonary hypertension. We studied 9 infants with severe respiratory failure characterized by hypoxemia and pulmonary hypertension. All infants met ECMO criteria and the oxygenation index (OI) was greater than 25. Mean birth weight was 2698 ± 661 g and gestational age was 36.4 ± 2.6 weeks. Nitric oxide was administered in a Y circuit in the inspiratory line of the mechanical ventilator. Nitric oxide and NO2 concentrations were monitored with electrochemical analyzers (PACI and PACII-Draeger). Pulmonary hypertension was diagnosed with clinical and echocardiografic criteria, with detection of right to left shunt with color doppler. All patients showed a dramatic improvement in oxigenation after nitric oxide administration. The drug reduced the mean OI, which was 48.5 before its administration, to 17.7 after 30', 14.1 after 6 hours, and 10.5 after 12 hours. We observed in all patients a reduction in pulmonary vascular resistance, reversal of the right to left shunt without any effects on systemic arterial pressure. Metahemoglobin levels did not reach 1.5% in any patient. Only one out of the 9 patients died, after reversal of the pulmonary hypertension, from other complications of perinatal asphyxia. Our data show that nitric oxide is a promising drug in the treatment of neonatal pulmonary hypertension and that it may reduce the need of ECMO in severe respiratory failure. | |
