dc.description | Objetivo: Avaliar mecanismos de morte na doença de Chagas (DC) correlacionando com a função ventricular. Métodos: Estudo prospectivo, longitudinal em coorte de 1053 pacientes com DC no período de 03/1990 a 03/2003. Todos foram submetidos a exame clínico, ECG, RX tórax e ECO na admissão. Os óbitos de causa cardíaca decorreram de morte súbita (MS), insuficiência cardíaca (IC) ou evento cardioembólico (AVE). Na análise estatística, utilizaram-se os testes qui-quadrado, t de Student e ANOVA (correção de Bonferroni). Resultados: O tempo médio de acompanhamento foi de 66±43 meses (seguimento completo de 84%). Ocorreram 93 óbitos, 78 (83%) de causa cardíaca: 53 MS, 20 IC e 5 AVE. Ocorreram 6 óbitos (causa nãocardíaca) quando o ECG foi normal e 87 quando foi anormal, 78 de causa cardíaca. MS foi o mecanismo mais freqüente de óbito em todas as faixas de fração de ejeção (FE). Morte por IC só ocorreu na FE <35% com 83% dos óbitos na classe funcional III ou IV com tratamento clínico otimizado. A FE média foi de 25±7% nos que faleceram de IC, 38±16% naqueles com MS; 51±19% naqueles com AVE embólico e de 56±14% nos pacientes com morte não-cardíaca (p<0,0001). Conclusões: Morte cardíaca ocorreu em 83% dos óbitos. MS foi o mecanismo geral de morte mais freqüente em qualquer faixa de FE. Morte por IC só ocorreu na FE <35% em classe funcional III ou IV com tratamento clínico otimizado. AVE presumivelmente embólico foi um mecanismo de morte significativo nos pacientes com função sistólica preservada ou levemente deprimida. | |
dc.description | Objective: To evaluate death mechanisms in Chagas disease correlating them with ventricular function. Methods: Prospective longitudinal study of a cohort of 1053 Chagas disease patients between March 1990 and March 2003 submitted to clinical examination, ECG, chest X-ray and Echocardiogram on admission. The deaths by cardiac cause were due to sudden death (SD), cardiac insufficiency (CI) or a cardio embolic event. Statistical analysis employed the chi square test, Student’s T-test, and ANOVA (Bonferroni correction). Results: Mean time of follow-up was 66±43 months, (complete follow-up rate of 84%) during which there were 93 deaths: 78 (83%) from cardiac cause: 53 SD, 20 CI, and 5 deaths from encephalic vascular accident (stroke); 6 deaths (non-cardiac cause) occurred among patients with normal electrocardiograms and in 87 patients with abnormal electrocardiograms; 78 of which from cardiac causes. SD was the most frequent death mechanism in all considered ejection fraction (EF) ranges. Death from CI occurred only in patients with EF <35% with 83% of the mortality in functional class III or IV after optimized clinical treatment. Mean ejection fraction was 25±7% in the patients who died from CI, 38±16% in those with SD, 51±19% in those with embolic stroke, and 56±14% in the patients with non-cardiac death (p<0.0001). Conclusions: Cardiac death caused 83% of the deaths. SD was the most frequent death mechanism for any given ejection fraction. Death from CI only occurred in EF <35% in functional class III or IV after optimized clinical treatment. Presumably embolic stroke was a significant death mechanism among the patients with either preserved or slightly depressed systolic function. | |
