Validez y confiabilidad de la escala colombiana de suicidio en español, para el tamizaje de riesgo suicida en adultos

dc.descriptionspa:Antecedentes: Las crecientes tasas de suicidio y la escasez de recursos al nivel del hemisferio sur requieren herramientas precisas, breves y gratuitas para facilitar y acelerar la detección. Un buen candidato es la Escala de Suicidalidad (SS) de 8 ítems con licencia Creative Commons derivada de la Escala Suicidal Affect-Behavior-Cognition (SABCS), una escala válida basada en la teoría con alta capacidad predictiva. Métodos y resultados: la validación y la evaluación de las propiedades psicométricas de la escala se llevó a cabo en Colombia en un estudio transversal prospectivo, con una muestra mixta de pacientes psiquiátricos y miembros de la comunidad sanos (N = 313, 132 mujeres, de 18 a 65 años) . Múltiples modelos (por ejemplo, análisis bifactorial, IRT) examinaron la unidimensionalidad, la consistencia interna y la validez. El sesgo del instrumento se probó por sexo, edad y diagnóstico psiquiátrico. Todos los modelos mostraron un fuerte ajuste y un bajo error para el SS. La consistencia interna fue alta (ω = .96). Las decisiones de riesgo de suicidio clínico se correlacionaron fuertemente con las puntuaciones de SS (r = 0,84); sin embargo, hubo una variación significativa en las tendencias suicidas en cada nivel de riesgo clínico. Conclusiones: Estos resultados proporcionan una fuerte validación para la Escala de Suicidio Colombiano-Español. El SS funcionó bien tanto en el modo de autoinforme como en el modo administrado por el médico; sin embargo, se requieren más estudios longitudinales para mejorar la orientación clínica sobre el riesgo de suicidio.

A good candidate is the Creative Commons licensed 8-item Suicidality Scale (SS) derived from the Suicidal Affect-Behavior-Cognition Scale (SABCS), a valid, theory-grounded scale with high predictive ability. Methods & Findings: Spanish translation, validation and evaluation of the scale’s psychometric properties was carried out in Colombia in a prospective transversal study, with a mixed sample of psychiatric patients and healthy community members (N = 313, 132 female, aged 18-65). Multiple models (e.g., bifactor analysis, IRT) examined unidimensionality, internal consistency, and validity. Instrument bias was tested by sex, age, and psychiatric diagnosis. All models showed strong fit and low error for the SS. Internal consistency was high (ω = .96). Clinical suicide risk decisions correlated strongly with SS scores (r = .84), however, there was significant variance in suicidality at each clinical risk level. Conclusions: These results provide strong validation for the Colombian-Spanish Suicidality Scale. The SS functioned well in both self-report and clinician-administered modes, however, further longitudinal study is required to improve clinical guidance on suicide risk.
