dc.descriptionspa:Introducción: La Osteoartritis es una enfermedad crónica, progresiva, prevalente, con un alto impacto en los sistemas de salud y en la calidad de vida de los pacientes y familiares. El dolor y la limitación funcional son los síntomas más comunes, por lo que se han propuesto diferentes alternativas novedosas en su tratamiento, derivadas del uso de células mesenquimatosas intraarticulares que ayudan a reducir el impacto en su patogenia. Objetivo general: Evaluar la calidad de la evidencia sobre la eficacia clínica y radiológica del uso de las células mesenquimatosas intraarticulares en comparación con otros tratamientos intraarticulares (placebo, corticoesteroides y ácido hialurónico intraarticular) contra la osteoartritis de rodilla de cualquier etiología para pacientes adultos, reportada entre 2016 y 2021. Materiales y métodos: Se realizó una revisión sistemática en las bases de datos Science Direct, Scielo, Clinical Trials, Web of Science, OVID, SCOPUS, PubMed (Medline), Cochrane, Biblioteca Virtual en salud, Google Scholar de artículos publicados en inglés y español, entre el 1 de enero de 2016 y el 22 de agosto de 2021. Estudios que han evaluado la eficacia clínica y radiológica de las células mesenquimatosas intraarticulares en comparación con otros tratamientos intraarticulares (placebo, corticoesteroides y ácido hialurónico intraarticulares) contra la osteoartritis de rodilla. Resultados: Se obtuvieron veintidós ensayos clínicos entre estudios aleatorizados y no aleatorizados; para el análisis final con un número total de pacientes inyectados con las células madre que oscilaron entre 6 y 57. Un total de 422 pacientes fueron tratados con una variedad de células madre mesenquimales, y un total de 246 de los pacientes sirvieron como controles. En todos los estudios se incluyeron pacientes con índice de masa corporal por debajo de 35 kg/m2 y con artrosis Kellgren – Lawrence que oscilaba entre grado I a IV. Conclusiones: A pesar de la mejoría demostrada en la función articular y dolor con la aplicación de MSC intraarticular en un seguimiento a corto plazo y con marco muestral reducido, por el alto riesgo de sesgo de los artículos publicados actualmente y su baja calidad metodológica, antes de recomendar este tratamiento en la práctica clínica cotidiana se debe evaluar su eficacia y seguridad de manera más rigurosa.
