Triagem auditiva e avaliação do equilíbrio em idosos com histórico de traumatismo cranioencefálico

Abstract

Objetivo: Realizar a triagem auditiva e avaliação do equilíbrio estático e dinâmico em indivíduos idosos com histórico de traumatismo cranioencefálico. Métodos: Foram selecionados pacientes em acompanhamento no Ambulatório de Neurotrauma/Neurocirurgia da Universidade Federal de São Paulo. Os indivíduos foram submetidos à história clínica, meatoscopia, questionário Dizziness Handicap Inventory, triagem auditiva nas frequências de 1000, 2000 e 4000 Hz e avaliação do equilíbrio estático (testes de Romberg clássico e sensibilizado) e dinâmico (Timed Up and Go test). Resultados: A triagem auditiva revelou que do total da amostra (18), 50% falharam à orelha direita e 72,22% à orelha esquerda. A média da nota pré trauma foi de 8,67 e de 7,7 pós trauma, sendo assim, houve diferença entre a nota atribuída pelo participante em relação à audição pré e pós traumatismo cranioencefálico. Quanto ao equilíbrio, no teste de Romberg Clássico a maioria da amostra apresentou um resultado negativo (94,12%), ou seja, sem alteração. Já no Romberg Sensibilizado, o número de resultados positivos foi de 62,50%. No teste Timed Up and Go, 29,40% dos indivíduos realizaram a tarefa em um tempo maior que 12,47 segundos, indicando risco para queda. Em relação à aplicação do questionário Dizziness Handicap Inventory, seis participantes (33%) pontuaram acima de zero, indicando alguma interferência da tontura em sua qualidade de vida. Conclusão: Idosos pós traumatismo cranioencefálico apresentam falhas na triagem auditiva em ambas as orelhas, com resultados piores à orelha esquerda revelando configuração descendente. Também apresentam alterações dos equilíbrios estático e dinâmico, com risco de quedas e impacto da queixa de tontura na qualidade de vida.

Objective: To perform hearing screening and assessment of static and dynamic balance in older adults after traumatic brain injury. Method: Patients were selected at Neurotrauma/Neurosurgery at Hospital São Paulo of UNIFESP, after being approved by the institutional ethics committee. Individuals were submitted to clinical history, meatoscopy, Dizziness Handicap Inventory questionnaire, hearing screening at frequencies of 1000, 2000 and 4000 Hz and assessment of static (classical and sensitized Romberg tests) and dynamic (Timed Up and Go test) balance. Results: Hearing screening revealed that of the total sample (18), 50% failed to the right ear and 72,22% to the left ear. The average score before trauma was 8,67 and 7,7 after trauma, thus, there was a difference between the score attributed by the participant to hearing before and after traumatic brain injury. For balance results, in the Classical Romberg test most of the sample showed a negative result (94,12%), that is, without alteration. In the Sensitized Romberg, the number of positive results was 62,50%. In the TUG test, 29,40% of the individuals performed the task in a time greater than 12,47 seconds, indicating a risk of falling. Regarding the application of the Dizziness Handicap Inventory questionnaire, six participants (33%) scored above zero, indicating some interference of dizziness in their quality of life. Conclusion: Older adults after traumatic brain injury had hearing screening failures in both ears, with worse results in the left ear and revealing a descending configuration. They also presented changes in static and dynamic balance, with risk of falls and impact of dizziness on quality of life.
