A interdependência entre as residências em saúde - SUS e a educação permanente em saúde municipais, Região Metropolitana da Baixada Santista

Abstract

O presente trabalho pretendeu analisar as ações desenvolvidas pelo setor de Educação Permanente em Saúde (EPS), entre elas as relacionadas às Residências em Saúde, na Atenção Primária em Saúde (APS), especificamente na Estratégia de Saúde da Família (ESF), nos municípios da Região Metropolitana da Baixada Santista (RMBS). Foi traçado um paralelo entre a trajetória nacional do desenvolvimento da Política Nacional de Educação Permanente (PNEPS), suas diretrizes e o desenvolvimento da EPS e Residências nos municípios da região. Também analisou-se sob o prisma do planejamento dessas áreas tratadas no Plano Municipal de Saúde (PMS) quadriênio 2018-2021. A combinação dessas análises permitiu a compreensão do papel de gestão da educação pela EPS municipal e a relação de interdependência com a Residências em Saúde, que objetiva a formação no SUS para o SUS. Objetivo: Analisar como se dá a relação entre a gestão da EPS e as Residências municipais na RMBS, e quais os impactos no desenvolvimento de ambas. Método: Para atingir o objetivo optou-se pela pesquisa quali-quantitativa com uso de entrevista e narrativa de si, feita pela pesquisadora, com os gestores das EPS de 5 municípios, somados representaram as vivências da relação entre as EPS e Residências. Complementar aos achados das entrevistas, a pesquisa documental do PMS desses municípios, na busca por diretrizes e eixos referente às áreas de EPS e Residências, planejadas e executadas no quadriênio 2018-2021. Resultados: As EPS pesquisadas dos municípios da RMBS, estão estruturadas junto a gestão central das secretarias de saúde, atuando nas ações educacionais, de ensino e serviço, pesquisa e formação. Apresentam perfil transversal e intersetorial, com equipes enxutas e em papel de intensa interlocução com atores internos e externos. Apontada as EPS como as responsáveis pela implantação das Residências em Saúde, sendo fundamental na sua construção e desenvolvimento. Com apoio dos gestores para a sua implantação, as Residências trouxeram maior visibilidade para as EPS, ampliando as ações educativas nas unidades que tem residentes. Na relação das EPS com as Residências todos os municípios apresentam papéis comuns como: apoio logístico, secretaria acadêmica, alimentação dos sistemas dos Ministérios da Educação e da Saúde, interlocução com as unidades de saúde. Algumas EPS, exercem também o papel de apoio pedagógico ao núcleo docente, recrutamento e capacitação dos preceptores, apoio nos estágios externos, entre outros. Em alguns municípios a expansão e fortalecimento da EPS levou a implantação das Residências, enquanto para um deles a implantação das Residências levou a formalização e fortalecimento da EPS. Entre a formalização hierárquica e a implantação das Residências aconteceram em momentos próximos, uma impulsionando a outra. Considerações finais: Evidenciada a relação de interdependência entre as EPS e as Residências, como também a sobrecarga de trabalho, nas EPS nas diversas ações desenvolvidas, é responsável pela estrutura de apoio na formação dos residentes dentro da esfera municipal, nas Residências pela intensa atividade acadêmica e de preceptoria. Aponta que para o crescimento e manutenção das Residências existe a necessidade de ampliação das equipes de EPS. Sugerido pelos entrevistados a adoção de estratégias de municipal, regional, e das universidades para o fortalecimento das EPS e apoio aos que pretendem implantar as Residências em Saúde.

The present work intended to analyze which actions were developed by the Permanent Education in Health (EPS) sector, including those related to Residencies in Health, in the APS, specifically in the Family Health Strategy (ESF), in the municipalities of the Metropolitan Region of Baixada Santista (RMBS). Drawing a parallel between the national trajectory of the development of the National Policy for Permanent Education (PNEPS), its guidelines and the development of EPS and Residences in the municipalities of the region. Complement this information to analyze from the perspective of the planning of these areas treated in the Municipal Health Plan (PMS) four-year period 2018-2021. The combination of these analyzes allowed understanding the role of education management by the municipal EPS and the relationship of interdependence with the Residencies in Health, which aims at training in the SUS for the SUS. Objective: To analyze how the relationship between EPS management and municipal residences in the RMBS takes place, and what are the impacts on the development of both. Method: To achieve the objective, we opted for qualitative and quantitative research using an interview and self-narrative, made by the researcher, with EPS managers from 5 municipalities, together representing the experiences of the relationship between EPS and Residences. Complementing the findings of the interviews, the documentary research of the PMS of these municipalities, in the search for guidelines and axes related to the areas of EPS and Residences, planned and executed in the four-year period 2018-2021. Results: The EPS surveyed in the municipalities of the RMBS are structured along with the central management of the health departments, working in educational, teaching and service, research and training actions. They have a cross-sectional and intersectoral profile, with lean teams and on a role of intense dialogue with internal and external actors. EPS are pointed out as responsible for the implementation of Health Residences, being fundamental in their construction and development. With the support of managers for its implementation, the Residences brought greater visibility to the EPS, expanding educational actions in the units that have residents. In the relationship between EPS and Residences, all municipalities have common roles, such as: logistical support, academic secretariat, supplying the Ministry of Education (MEC) and Ministry of Health (MS) systems, dialogue with health units. Some EPS also play the role of pedagogical support to the teaching staff, recruitment and training of preceptors, support in external internships, among others. In some municipalities, the expansion and strengthening of the EPS led to the implementation of Residences, while for one of them the implementation of Residences led to the formalization and strengthening of the EPS. Between the hierarchical formalization and the implementation of the Residencies happened in close moments, one driving the other. Final considerations: Having evidenced the relationship of interdependence between the EPS and the Residences, as well as the work overload, in the EPS in the various actions developed, it is responsible for the support structure in the training of residents within the municipal sphere, in the Residences for the intense academic activity and preceptorship. It points out that for the growth and maintenance of the Residences there is a need to expand the EPS teams. The interviewees suggested the adoption of municipal, regional and university strategies to strengthen EPS and support those who intend to implement Health Residencies.
