Cuidados paliativos en la Unidad de Cuidados Intensivos Hospital de tercer nivel de la ciudad de Guayaquil

Abstract

La Unidad de Cuidados Intensivos (UCI) es un sector de atención médica de alta complejidad que tiene por objetivo beneficiar a los pacientes en situación crítica, estabilizando las variables clínicas y hemodinámicas esenciales por eso los cuidados paliativos son de vital importancia dentro de esta área porque pretenden mejorar la calidad de vida de las personas con enfermedades graves (degenerativas o terminales) que no responden al tratamiento curativo. Objetivo: generar un algoritmo para cuidados paliativos en pacientes críticos a partir de la evidencia clínica para una mejor calidad de vida durante su estancia en las UCI. Diseño: investigación de diseño retrospectivo de enfoque cuantitativo y de tipo descriptivo. Se incluyeron a los pacientes ingresados en la Unidad de Cuidados Intensivos de un Hospital de Tercer Nivel de la Ciudad de Guayaquil durante el periodo diciembre 2020 -diciembre 2021. Resultados: la mayoría de los ingresos en la UCI correspondieron al sexo femenino (50,74) y a ancianos mayores de 65 años (64,17%), la causa más común de internación fue la cardiovascular con el 40%, la gran mayoría fue dada de alta (80,59%) sin embargo de los fallecimientos el gran porcentaje fue en los mayores de 65 años (84.61%); el 44,77% de los pacientes estuvo en el rango comprendido como “estancia media” es decir de 5 a 13 días. Conclusión: Los cuidados paliativos son parte fundamental del manejo de toda enfermedad grave o en fase terminal y deben ser atendidos con la mayor evidencia científica. El diagnóstico de ingreso, las causas de fallecimiento son líneas transversales a tomar en consideración a lo largo del cuidado durante el tiempo de vida, brindan el parámetro para explorar las necesidades básicas y vitales de los pacientes.

Design: retrospective design research with a quantitative and descriptive approach. Patients admitted to the Intensive Care Unit of a Third Level Hospital in the City of Guayaquil during the period December 2020 -December 2021 were included. Results: the majority of ICU admissions were female (50.74 ) and elderly people over 65 years of age (64.17%), the most common cause of hospitalization was cardiovascular with 40%, the vast majority were discharged (80.59%), however, of the deaths the large percentage was in those over 65 years of age (84.61%); 44.77% of the patients were in the range understood as "average stay", that is, from 5 to 13 days. Conclusion: Palliative care is a fundamental part of the management of any serious or terminal illness and must be attended with the best scientific evidence.
