dc.description.abstract | La neutropenia febril es la complicación más común y grave asociada con los cánceres
hematopoyéticos o con pacientes que reciben regímenes quimioterapéuticos para el
cáncer, ocurre cuando un paciente neutropénico se encuentra con un patógeno
infeccioso. La administración de agentes antibacterianos de amplio espectro, la
hospitalización prolongada, las infecciones asociadas a la atención médica, la
presencia de un catéter urinario se consideran factores de riesgo importantes para la
aparición de bacterias resistentes. OBJETIVO: Determinar la relación entre el perfil de
resistencia bacteriana con la mortalidad en pacientes oncohematológicos
neutropénicos febriles posquimioterapia del Instituto Oncológico “Juan Tanca
Marengo”, en el período comprendido de Enero a Diciembre 2018. METODOLOGÍA: El
estudio fue de tipo observacional, descriptivo, retrospectivo y de corte transversal.
RESULTADOS: Se analizaron 181 pacientes; el 65,7% fue de género femenino; edad
media de 48,3 años; el diagnóstico oncológico de mayor porcentaje fue el Cáncer de
mama con el 34,8%; la sepsis urinaria con el 45,3%, representó el sitio de infección de
mayor porcentaje, el 35,9% falleció; el valor medio absoluto de neutrófilo fue de 154 (p:
0,05), los pacientes con neutropenia febril posquimioterapia que fallecieron fueron
aquellos cuyos cultivos reportaron crecimiento de Escherichia coli en el 14,4% (p:
0,001), reportando perfil de resistencia bacteriana con cepa productora de BLEE en el
15,5% (p: 0,001), seguido del 8,3% de cepa productora de carbapenemasa, además el
22,7% (p: 0,00), estuvo hospitalizado previamente, el 23,8% (p: 0,00) recibieron antibiótico previamente. CONCLUSIONES: La bacteria identificada en su mayoría fue la Escherichia coli, cepa productora de BLEE, falleciendo los pacientes que tuvieron valor medio absoluto de neutró filo de 154, al igual que aquellos que fueron
hospitalizados y han recibido antibióticos previamente. | |
dc.description.abstract | Febrile neutropenia, the most common and serious complication associated with
hematopoietic cancers or with patients receiving cancer chemotherapeutic regimens,
occurs when a neutropenic patient encounters an infectious pathogen. The
administration of broad-spectrum antibacterial agents, prolonged hospitalization,
infections associated with medical care, the presence of a urinary catheter are
considered important risk factors for the appearance of resistant bacteria. OBJECTIVE:
To determine the relation ship between the bacterial resistance profile and mortality in
post-chemotherapy febrile neutropenic oncohematological patients of the "Juan Tanca
Marengo" Oncology Institute, in the period from January 2018 to December 2021.
METHODOLOGY: The study was observational, descriptive, retrospective and cross sectional. RESULTS: 181 patients were analyzed, 65.7% were female; mean age 48.3
years; the oncological diagnosis with the highest percentage was breast cancer with
34.8%; urinary sepsis with 45.3% represented the site of infection with the highest
percentage, 35.9% died; the absolute mean value of neutrophils was 154 (p: 0.05), the
patients with post - chemotherapy febrile neutropenia who died were those whose
cultures reported growth of Escherichia coli in 14.4% (p: 0.001), reporting a profile of
bacterial resistance with ESBL- producing strain in 15.5% (p: 0.001), followed by 8.3%
carbapenemase - producing strain, in addition 22.7% (p: 0.00) had been previously
hospitalized, 23.8% (p: 0.00) previously received an antibiotic. CONCLUSIONS: The
majority of the bacteria identified was Escherichia coli, a ESBL - producing strain, with
patients who had an absolute mean neutrophil value of 154 dying, as well as those who
were hospitalized and had previously received antibiotics | |
