Efectividad analgésica del metamizol más tramadol versus metamizol más codeina para el manejo del dolor post operatorio en colecistectomía laparoscópica

OBJETIVO: El objetivo fue determinar la efectividad analgésica del Metamizol
más Tramadol versus Metamizol más Codeína en el manejo del dolor
postoperatorio en pacientes con colecistectomía laparoscópica.
MATERIAL Y MÉTODOS: Se realizó un estudio observacional, analítico, de
cohortes prospectivo, en el Hospital II Huánuco, en el periodo de Octubre 2016 a
Junio 2017. Fueron evaluados 108 pacientes, quienes fueron distribuidos en 2
grupos, el grupo I 54 pacientes que recibieron metamizol más tramadol y el grupo
II 54 pacientes quienes recibieron metamizol más codeína como tratamiento de
dolor postoperatorio.
RESULTADOS: La edad promedio en los grupos I y II fueron (43 ± 12 vs 44,5 ±
14,2 años; p>0.05), la proporción del sexo masculino en los grupos I y II fueron
38,9% vs 40,7%; p>0.05. La evaluación del dolor utilizando la escala visual
análoga (EVA) obtuvo los siguientes promedios: a las 2 horas (1,93 vs 4,87;
p<0.05), 4 horas (1,68 vs 3,46; p>0.05), 6 horas (0,63 vs 1,16; p>0.05) y 12 horas
(0,31 vs 0,15; p>0.05). La terapia de rescate fue necesaria en 12 (11.1%)
pacientes del grupo I y en 33 (30.6%) del grupo II (p<0.05). Respecto a las
reacciones adversas (náuseas, vómitos, mareos) no se encontraron diferencias
entre los grupos (p>0.05).
CONCLUSIONES: El Metamizol más Tramadol tiene mejor efecto analgésico
hasta las 2 horas y tiene menos uso de terapia de rescate en comparación al
Metamizol más Codeína; sin embargo, con estos hallazgos no se puede concluir
que el metamizol más tramadol tenga mejor efectividad analgésica. OBJECTIVE: The objective was to determine the analgesic effectiveness of
Dipyrone plus Tramadol versus Dipyrone plus Codeine in the management of
postoperative pain in patients with laparoscopic cholecystectomy.
MATERIAL AND METHODS: An observational, analytical, prospective cohort
study was conducted in the Hospital II Huánuco, in the period from October 2016
to June 2017. 108 patients were evaluated, who were divided into 2 groups, the
group I 54 patients Dipyrone plus tramadol and group II received 54 patients who
received Dipyrone plus codeine as a treatment for postoperative pain.
RESULTS: The average age in groups I and II were (43 ± 12 vs 44.5 ± 14.2 years,
p> 0.05), the proportion of males in groups I and II were 38.9% vs 40.7% p> 0.05.
The evaluation of the pain using the visual analogue scale (VAS) obtained the
following averages: at 2 hours (1.93 vs 4.87, p <0.05), 4 hours (1.68 vs 3.46, p>
0.05) , 6 hours (0.63 vs 1.16, p> 0.05) and 12 hours (0.31 vs 0.15, p> 0.05).
Rescue therapy was necessary in 12 (11.1%) patients in group I and 33 (30.6%) in
group II (p <0.05). Regarding the adverse reactions (nausea, vomiting, dizziness)
no differences were found between the groups (p> 0.05).
CONCLUSIONS: Dipyrone plus Tramadol has better analgesic effect until 2 hours
and has less use of rescue therapy compared to Dipyrone plus Codeine; However,
with these findings it can not be concluded that Dipyrone plus tramadol has better
analgesic effectiveness.
