dc.description.abstract | Objetivo: Delimitar si el Índice Child Pugh + Lactato tiene una sensibilidad mayor al
80% como predictor de muerte en pacientes con cirrosis hepática hospitalizados en
el servicio de Gastroenterología del Hospital Regional Docente de Trujillo, 2012 –
2017.
Material y Método: Se llevó a cabo un estudio de prueba diagnóstica, retrospectivo
y observacional en una población de 121 pacientes diagnosticados con cirrosis
hepática y hospitalizados en el servicio de Gastroenterología del Hospital Regional
Docente de Trujillo durante el periodo Enero 2012 – Enero 2017. Se calculó la
sensibilidad, especificidad, valores predictivos, exactitud diagnóstica; así como el
punto de corte de dicho índice. Se aplicó el test de Chi cuadrado, T- Student y Área
bajo la curva.
Resultados: De los 121 pacientes incluidos en el estudio, fallecieron un total de 40
(33%) y sobrevivieron 81 (67%). El punto de corte del índice Child Pugh + Lactato
como predictor de muerte en pacientes con cirrosis hepática fue ≥ 11 puntos; con
una sensibilidad, especificidad, valor predictivo positivo y negativo de 87,5%, 49,4%,
46,1% y 88,9% respectivamente. La exactitud diagnóstica del mismo fue de 70,5%.
El promedio del índice Child Pugh + Lactato fue mayor en el grupo de pacientes
cirróticos fallecidos, este presentó significancia estadística (p=0.002).
Conclusiones: El Índice Child Pugh + Lactato no posee valor como predictor de
muerte en pacientes con cirrosis hepática. | |
dc.description.abstract | Objective: Delimit if the Child Pugh + Lactate Index has a greater sensitivity to 80%
as a predictor of death in patients with liver cirrhosis hospitalized in the
Gastroenterology service of the Regional Teaching Hospital of Trujillo, 2012 - 2017.
Material and Method: A retrospective and observational diagnostic test study was
carried out in a population of 121 patients diagnosed with liver cirrhosis and
hospitalized in the Gastroenterology service of the Regional Teaching Hospital of
Trujillo during the period January 2012 - January 2017. Sensitivity was calculated,
specificity, predictive values, diagnostic accuracy; as well as the cutoff point of said
index. The Chi-square, T-Student and Area under the curve test were applied.
Results: Of the 88 patients included in the study, a total of 40 (33%) died and 81
survived (67%). The best cut-off point of the Child Pugh + Lactate index as a
predictor of death in patients with liver cirrhosis was ≥ 11 points; with a sensitivity,
specificity, positive and negative predictive value of 87,5%, 49,4%, 46,1% and 88,9%
respectively. The diagnostic accuracy of the same was 70,5%. The Child Pugh +
Lactate index was higher in the group of deceased cirrhotic patients, it did present
statistical significance (p= 0.002).
Conclusions: The Child Pugh + Lactate Index has no value as a predictor of death
in patients with liver cirrhosis. | |
