dc.description.abstract | Introducción: El lupus Eritematoso sistémico es una enfermedad de tipo autoinmune la
cual causa múltiples sintomatologías y afecta a diversos sistemas. Por tal motivo es
importante determinar los factores relacionados a la progresión y supervivencia de la
enfermad. Objetivo: Conocer la asociación de los factores sociodemográficos, clínicos y
ambientales con la progresión y supervivencia de los pacientes con LES en ESSALUD y
MINSA - Cusco del 2019 – 2022. Material y Método: El estudio es de tipo correlacional
asociativo, retrospectivo. El muestreo fue por conveniencia no probabilístico teniendo en
cuenta los criterios de selección de los pacientes. Se recopilo una muestra de 91 pacientes
y se analizó los datos en el programa SPSS 21. Resultados: Resultaron asociados a una
mayor progresión del LES: Malestar General, Comorbilidades urinarias, TVP, Síndrome
de Raynaud, pleuritis, Proteinuria, hematuria, Síndrome Nefrótico, cilindros celulares,
neutropenia, linfopenia, trombocitopenia, presencia de Nefropatía terminal, ANA +, uso
crónico de AINEs. Con respecto al análisis de supervivencia se encontró una mortalidad
a los 10, 20, 30 y más de 30 años de su diagnóstico fue 3%, 10%, 18% y 100%
respectivamente. La principal causa de muerte fueron las infecciones, la segunda causa la
actividad lúpica y tercera causa el COVID- 19. Además del resultado de la asociación
entre variables clínicas y supervivencia se evidencia, que los pacientes que presentaron
Pleuritis; efusión pleural, Nefropatía terminal; trombocitopenia, Deformidad de articulaciones, alopecia y esplenomegalia se asociaron a una menor supervivencia de
LES.
Conclusiones: Las principales comorbilidades y la principal causa de muerte fueron las
infecciones, cuyo origen mayoritario fue en las vías urinarias. Las manifestaciones
clínicas pulmonares y renales, la presencia de trombocitopenia, deformidad de
articulaciones y alopecia, se asociaron a una menor supervivencia, así como también los
pulsos de metilprednisolona y FR + se evidencio asociación con la supervivencia. Las
infecciones como COVID 19 y de las vías urinarias, así como la presencia
manifestaciones renales, cutáneas (fenómeno Raynaud) y cardiovasculares (TVP) se
asociaron a una mayor progresión de la enfermedad. | |
dc.description.abstract | Introduction: Systemic lupus erythematosus is an autoimmune disease that causes
multiple symptoms and affects various systems. For this reason, it is important to
determine the factors related to the progression and survival of the disease. Object: To
know the association of sociodemographic, clinical, and environmental factors with the
progression and survival of patients with SLE in ESSALUD and MINSA - Cusco from
2019 – 2022. Methods: Our study is retrospective, associative correlational type.
Sampling was for non-probabilistic convenience, considering the patient selection
criteria. A sample of 91 patients was collected. For data analysis, we used the SPSS 21
program. Results: They were associated with a greater progression of SLE: General
malaise, urinary comorbidities, DVT, Raynaud's syndrome, pleurisy, proteinuria,
hematuria, nephrotic syndrome, cellular casts, neutropenia, lymphopenia,
thrombocytopenia, presence of end-stage renal disease, ANA +, chronic use of NSAIDs.
Regarding the survival analysis, mortality, was found at 10, 20, 30 and more than 30 years
after diagnosis was 3%, 10%, 18% and 100% respectively. The main cause of death was
infections, the second cause lupus activity and the third cause COVID-19. In addition to
the result of the association between clinical variables and survival, it is evident that the
patients who presented Pleurisy; pleural effusion, End-stage renal disease;
thrombocytopenia, conjunctivitis, joint deformity, alopecia, and splenomegaly were
associated with lower SLE survival. Conclusions: The main comorbidities and the main cause of death were infections, the majority of which originated in the urinary tract.
Pulmonary and renal clinical manifestations, the presence of thrombocytopenia,
conjunctivitis, joint deformity, and alopecia were associated with lower survival, such as
pulsed methylprednisolone and FR have association with survival, it was evidenced.
Infections such as COVID 19 and those of the urinary tract, as well as the presence of
renal, cutaneous (Raynaud's phenomenon) and cardiovascular (DVT) manifestations,
were associated with a greater progression of the disease. | |
