Incidencia de traumatismo uretral iatrogénico a realizar en el Hospital General del Guasmo Sur en el período 01/01/2017 - 31/12/2017

Las lesiones por rotura de la uretra ocurren por lo general en combinación con politraumatismos en accidentes automovilísticos, caídas o accidentes industriales. La diástasis del pubis, las fracturas localizadas de las ramas pubianas o las fracturas pélvicas más complejas, pueden asociarse con rotura de la uretra. Las fracturas en silla de montar, expuestas o las lesiones que provocan inestabilidad pelviana vertical y de rotación, se asocian con el riesgo más alto de lesión urológica. Dado que la uretra posterior está sujeta tanto al diafragma urogenital como a los ligamentos puboprostáticos, la unión bulbomembranosa es más vulnerable a la lesión en las fracturas de la pelvis.
La sección de la uretra se evidencia a la exploración física con la tríada de: sangre en el meato, la incapacidad para orinar y la vejiga llena. Cuando se descubre sangre a nivel del meato uretral, se debe realizar de inmediato un uretrograma retrógrado para excluir lesión uretral.
En este trabajo que se realizó en el Hospital General Guasmo Sur, durante el periodo 01/01/2017 – 31/12/2017, se evidencia que los traumatismos uretral iatrogénicos en hospitalización es un 35% fueron atendidos por emergencia, 65% se registran en hospitalización lo que, reafirma que generalmente son cuadros severos que ameritan ingreso hospitalario.
Los traumas uretrales se pueden considerar como una patología casi exclusiva de los hombres, quienes en este trabajo tienen una frecuencia del 98%, y las mujeres apenas 2%. Los casos de trauma uretral encontrados como conclusión que es más frecuente 50% en los pacientes mayores de 65 años, 26% en pacientes de 20 a 49 años y de 50 a 64 años un 24%. Injuries due to rupture of the urethra usually occur in combination with
polytraumatisms in automobile accidents, falls or industrial accidents. Diastasis of
the pubis, localized fractures of the pubic branches or more complex pelvic fractures may be associated with rupture of the urethra. Saddle fractures, exposed or injuries that cause vertical pelvic instability and rotation, are associated with the highest risk of urological injury. Because the posterior urethra is attached to both the urogenital diaphragm and the puboprostatic ligaments, the bulbomembranous junction is more vulnerable to injury to pelvic fractures.
The section of the urethra is evidenced by physical examination with the triad of:
blood in the meatus, inability to urinate and full bladder. When blood is discovered
at the urethral meatus level, a retrograde urethrogram should be performed
immediately to exclude urethral injury.
In this work that was performed in the General Hospital of the South Guasmo,
ruante the period 2017 - 2018 it is evident that the urethral traumatisms in 33% were attended by emergency, 65% are registered in hospitalization which, reaffirms that they are generally severe pictures that warrant hospital admission, only 2% of patients were registered due to iatrogenesis.
Urethral traumas can be considered as an almost exclusive pathology of men, who in this work have a frequency of 99%, and women just 1%
