Dualidade pulsional e doenças autoimunes: reflexões de vida e morte.

Abstract

As Doenças Autoimunes são aquelas em que o sistema imunológico falha, atacando e destruindo células, tecidos ou órgãos saudáveis no próprio organismo, com causas desconhecidas para muitas das mais de cem dessas doenças já identificadas. Não há cura para tais doenças, apenas tratamentos que variam em sua eficácia e prejuízos colaterais, conforme a patologia, sua gravidade e quem adoece. Nesse sentido, este trabalho teve por objetivo compreender as manifestações pulsionais nas Doenças Autoimunes por intermédio do estudo de pessoas com Lúpus Eritematoso Sistêmico e Psoríase, abarcando duas participantes adultas, cada qual com uma das doenças referidas e ambas sem antecedentes genéticos de Doenças Autoimunes na família. Trata-se de uma pesquisa qualitativa, que se propôs a investigar aspectos do psiquismo humano que consideram a especificidade e subjetividade do indivíduo, mas que propiciem sistematizações de conhecimento acerca da generalidade da vida mental. Para tanto, a execução da pesquisa utilizou-se do Procedimento de Desenhos-Estórias com Tema, com quatro Unidades de Produção realizadas com cada participante, separadamente, buscando investigar aspectos essenciais de suas dinâmicas psíquicas, tais quais seus recursos defensivos e capacidade elaborativa do ego. Complementando o Procedimento, realizou-se uma Entrevista Semiestruturada, individualmente, visando resgatar aspectos do ciclo vital e vivências importantes que auxiliassem na compreensão do processo de adoecer e seus enfrentamentos. A análise e a sistematização dos dados se deram por meio do referencial teórico psicanalítico, visando relacionar a dualidade pulsional e as Doenças Autoimunes. Constatou-se, nas Unidades de Produção elaboradas, precariedade na representação e simbolização das participantes, bem como grande fragilidade psíquica, indicando cisões importantes. As verbalizações durante o Procedimento e nas entrevistas realizadas confirmam esses dados e nos possibilitam verificar a ocorrência de eventos traumáticos com rupturas significativas de vínculos primordiais na infância dessas participantes, que sugerem ter configurado a impossibilidade de manter o sofrimento psíquico em níveis mentais, sendo necessária a comunicação dessa dor avassaladora por meio do corpo somático. Por fim, compreende-se que, em alguns pacientes com Doenças Autoimunes, quando não há antecessores genéticos na família nem explicações ambientais em seu desencadeamento, a dualidade pulsional manifestou-se no corpo, caracterizando, ao mesmo tempo, tentativas de lidar com a dor e a busca pelo fim do sofrimento, tendo o corpo somático e o corpo das representações atuado juntos, defensivamente.

Autoimmune diseases are those in which the immune system fails, attacking and destroying healthy cells, tissues or organs in the body itself, with causes unknown to many of the more than one hundred of these diseases already identified. There is no cure for such diseases, only treatments that vary in their effectiveness and collateral damage, depending on the pathology, their severity and who gets sick. In this sense, this work aimed to understand the drive manifestations in Autoimmune Diseases, through the study of people with systemic lupus erythematosus and psoriasis, encompassing two adult participants, each of them with one of the mentioned diseases and both without genetic history of the disease in the family. This is a qualitative research, which aimed to investigate aspects of the human psyche that consider the specificity and subjectivity of the individual, but that provide systematizations of knowledge about the generality of mental life. To this end, for the execution of this research, it was used the Procedure of Drawings-Stories with Theme, with four Production Units performed by each participant, separately, aiming to investigate essential aspects of their psychic dynamics, such as their defensive resources and elaborative capacity of the ego. Data analysis and systematization occurred through the psychoanalytic theoretical framework, aiming to relate the drive duality and autoimmune diseases. In the production units elaborated it was found precarious representation and symbolization of the participants, as well as great psychic fragility, indicating important divisions. The verbalizations during the Procedure and in the interviews conducted confirm these data and allow us to verify that there were traumatic events with significant ruptures of primordial bonds in the childhood of these participants, which suggest that they have configured the impossibility of maintaining psychic suffering at mental levels, making the communication of this overwhelming pain through the somatic body necessary. Finally, we understand that in patients with autoimmune diseases, when there are no genetic predecessors in the family or environmental explanations in its triggering, the drive duality manifested itself in the body, characterizing, at the same time, attempts to deal with pain and the search for the end of suffering, having the somatic body and the body of representations acting together, defensively.
