Índice de pulsatilidad de arterias uterinas y tensión arterial media como método predictor de trastornos hipertensivos del embarazo

dc.descriptionObjetivo: Determinar la capacidad predictiva de desarrollo de trastornos hipertensivos del embarazo mediante la medición delíndice de pulsatilidad de las arterias uterinas y la tensión arterial media. Métodos: En 66 pacientes se determinó el índice de pulsatilidad de las arterias uterinas, la tensión arterial media y el índice de masa corporal entre 18 y 22 semanas más 6 días de gestación. Se consideró valor normal aquellos que se encontraban por debajo del percentil 95 para edad gestacional.Resultados: La tensión arterial media estuvo entre 73 y 83 mm Hg. El índice de pulsatilidad de las arterias uterinas fue normalen 97 %. Nueve pacientes presentaron trastorno hipertensivo del embarazo (13,6 %). La mayoría de estos casos se presentaronantes de las 37 semanas (7,6 % del total de la muestra). En el grupo que presentó el trastorno hipertensivo del embarazo, lamediana para la edad fue 2 años mayor, el índice de masa corporal fue un punto menor, la tensión arterial media tuvo unamediana similar al grupo sin la condición, pero con rango intercuartílico más amplio y elevado (P25=73 y P75=95 mm Hg).Conclusiones: Se encontró una correlación positiva entre alteración de las arterias uterinas y los trastornos hipertensivos delembarazo. De los factores de riesgo evaluados, la tensión arterial media presentó mejor comportamiento como factor predictor.El índice de pulsatilidad de las arterias uterinas fue mayor en el grupo que desarrolló trastorno hipertensivo del embarazo, consensibilidad de 11 %. Objective: To determine the predictive capacity of development of hypertensive disorders of pregnancy by measuring thepulsatility index of the uterine arteries and mean arterial pressure. Methods: In 66 patients, the pulsatility index of the uterine arteries, the mean arterial pressure and the body mass index between 18 and 22 weeks plus 6 days of gestation were determined. Those below the 95th percentile for gestational age were considered normal value. Results: Mean blood pressure was between 73 to 83 mm Hg. The pulsatility index of the uterine arteries was normal at 97%. Nine patients had hypertensive disorder of pregnancy (13.6 %). Most of these cases occurred before 37 weeks (7.6% of the total sample). In the group that presented with hypertensive disorder of pregnancy, the median for age was 2 years higher, body mass index was one point lower, mean blood pressure had a median similar to the group without the condition, but with wider and higher interquartile range (P25=73 and P75=95 mm Hg).Conclusions: A positive correlation was found between alteration of the uterine arteries and hypertensive disorders of pregnancy.Of the risk factors evaluated, mean blood pressure presented better behavior as a predictor. The pulsatility index of the uterinearteries was higher in the group that developed hypertensive pregnancy disorder, with sensitivity of 11%.es-ES
