| dc.description | En la actualidad el tratamiento quirúrgico de elección de la espondiloartrosis cervical degenerativa ha experimentado variaciones hasta la introducción de la Artrodesis Intersomática para la fusión cervical. Objetivo: Valorar la evolución clínica neurológica-radiológica de los pacientes con patología discal degenerativa que se sometieron a Artrodesis Intersomática Cervical Anterior y determinar su eficacia y utilidad. Métodos: se estudiaron 57 pacientes intervenidos en los últimos cinco años, hasta Diciembre del 2012, realizándose un análisis estadístico retrospectivo. A 40 pacientes se les implanto cesta dentada (Peek) sin placa y se utilizó cesta dentada (Peek) + placa en 17 pacientes. Se evaluó el dolor cervical, la cervicobraquialgia y la discapacidad funcional. Resultados: se observó mejoria de la cervicobraquialgia (84%) y (61%) de los trastornos sensitivos. El grupo etario mayor afectado fue entre los 40 y 50 años. El interespacio más afectado fue C5-C6 (65%) y entre los hallazgos radiológicos se observó: rectificación y pérdida de lordosis cervical y disminucion del espacio intervertebral. Se encontró en las complicaciones: disfonía, dolor postquirúrgico y disfagia. Sin embargo, se obtuvo una buena fusión en 47(82%) casos y solo 5(9%) presentaron extrusión discal y pseudoartrosis. Se apreció que el 90% de los pacientes se reincorporo a su actividad laboral. Conclusiones: la artrodesis cervical anterior sin placa es una opción válida de tratamiento. Sin embargo, la artrodesis intersomática con placa resulta una mejor opción terapéutica para esta patología, permitiendo una movilización precoz y un menor índice de complicaciones. | |
| dc.description | Degenerative disc disease: Treatment with Cervical Arthrodesis Anterior In the surgical treatment of choice for degenerative cervical spondylosis has experienced variations until the introduction of arthrodesis for cervical interbody fusion. Objective: To evaluate the clinical neurological and radiological outcome of patients with degenerative disc disease who underwent Anterior Cervical Interbody Arthrodesis and determine their effectiveness and usefulness. Methods: 57 patients treated in the last five years were studied, until December 2012, performing a retrospective statistical analysis 40 patients were implanted gear basket (Peek) without plate and notched Cart (Peek) + plate was used in 17 patients. Cervical pain, and functional disability cervicobrachialgia was evaluated. Results: improvement of cervicobrachialgia (84 %) and (61 %) of the sensory disturbance was observed. The largest age group affected was between 40 and 50. The most affected interspace was C5 -C6 (65%) and between radiological findings was noted: correction and loss of cervical lordosis and decrease the intervertebral space. It was found in the simple, dysphonia, dysphagia and postoperative pain. However, a good fusion in 47 (82%) cases and only 5 (9 %) had disc extrusion and pseudarthrosis was obtained. It was observed that 90 % of patients were reincorporated into their work .Conclusions: The anterior cervical fusion without plate is a valid treatment option. However, fusion with plate is a better treatment option for this disease, allowing early mobilization and a lower rate of complications. | |
