Objective. Validate the formulas of Cockcroft-Gault and MDRD (Modification of Diet in Renal Disease Study Group) versus creatinine clearance for the diagnosis and monitoring of the progression of renal failure. Materials and Methods. A study Validation Test. The study population was medical histories of 121 patients with acute or chronic renal failure presenting for diagnosis or monitoring of their condition at the Hospital José Carrasco of the Ecuadorian Institute of Social Security and private practice of clinical nephrology at Santa Inés Hospital Cuenca, Ecuador. It was considered the gold standard creatinine clearance compared to that validated the formulas of Cockcroft-Gault and MDRD for their sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio. Results. The sensitivity of the test was higher than Crockoft MDRD test but the difference was not significant. The predictive value of negative test (NPPV) in change was significantly greater for the test Crockoft. In the other analysis of both test values compared with the gold standard were similar and some even equal. The study population consisted of 64.4% (n = 78) of women and 35.5% (n = 43) of males with a mean age of 61.39 ± 17 years, 68% of the study population between 44.39 and 78.39 years. Conclusion. The analysis, in addition to the comparison with the gold standard, both formulas give similar goodness for the diagnosis of kidney failure, but with a higher sensitivity for the test Crockoft. Subject headings. Renal failure, Crockoft test, MDRD formula, creatinine clearance, validation of a test.
