The following investigation`s objective was the description of the clinical case study of a 50-year-old patient with a history of hyperthyroidism without regular treatment who comes to this health house with a marked intake of thyroid hormones, which is why it is diagnosed as picture of thyrotoxicosis, to the patient physical examination slightly disoriented, anxious, long-line structure, with marked tachycardia, tachypnea, with evident exophthalmos and tremors in upper limbs; BURCH and WARTOFSKY scale is applied with a score greater than 50 that confirms the diagnosis of thyroid storm, in addition complementary images are performed such as thyroid ultrasound that report enlarged thyroid size suggestive of diffuse goiter, electrocardiograms are performed where atrial fibrillation is evidenced with rapid ventricular response resistant to conventional medical treatment. Therapeutic management with anticoagulants is initiated as a preventive measure of thrombotic events, more heart rate controllers and later thyroid function control, which finally leads to a sinus rhythm that it is maintained in the subsequent controls by external consultation. Thyroid storm is a trigger and perpetuator of cardiac arrhythmias such as atrial fibrillation. Adequate control of thyroid function helps regulate the heart rate during the arrhythmic episode.
