Introduction: Extracorporeal shock waves reach the tissue surface providing various biological effects and may be beneficial to cellulite treatment, which involves changes in adipose tissue and microcirculation causing edema and fibrosis in the connective tissue. Objective: The purpose of the study was to evaluate the effects of shock wave therapy on the fibrous septa of cellulite in Brazilian women. Method: 20 women with cellulitis in the glutes (grade III), evaluated before and after treatment, submitted to evaluation protocols and ultrasound examinations. The volunteers were randomized divided into two subgroups: treatment group (GT) and control group (CG). The GT received Shockwave therapy, while in the GC, a vibratory massage was performed. The parameters used were: intensity of 3.5 bar, frequency 21 Hz and dose of 1,500 pulses per gluteal, both groups performed 6 treatment sessions, once a week, with 30 minutes). Results: The data were analyzed by the SPSS software, with a significance level of 5% and a 95% confidence interval. There was a significant reduction of GT septa in the right gluteus (p = 0.001) and left gluteus (p = 0.000). Only the right gluteus reduced the fat layer (p = 0.043). The CG did not present a reduction in the variables (p> 0.05). In the intergroup comparison, only the measurement of the thickness of the septum in the left gluteus showed a reduction generating a significant value (p = 0.008). Although no change in the other variables changed in any of the groups (p> 0.05), the participants reported improvement in quality of life. Conclusion: The shock wave protocol applied in this study significantly reduced the fat layer and cellulite septa.
