Resumen
Introduction: Melanin hyperpigmentation of the oral mucosa (OMH) is characterized by the deposition of pigments in the tissues, resulting from the increase in melanin production and/or an increase in the number of melanocytes. This clinical manifestation is one of the most frequent oral findings associated with the human immunodeficiency virus HIV, of which several agents, such as drugs used in antiretroviral therapy (ART), the action of certain antifungal and antibacterial drugs, dysregulation of the immune system, can contribute to its development. This study aims to explore, through a case report and review in the literature, the possible factors associated with a greater predisposition to OMH in patients with human immunodeficiency syndrome (AIDS). Case report: A 55-year-old male patient, living with HIV undergoing antiretroviral treatment, attended to the Stomatology service of the Federal University of Rio Grande do Norte (UFRN) with the chief complaint of “a purple spot on the cheek” without a history of painful symptoms. At intraoral clinical examination, the presence of two extensive macules in buccal mucosa was observed, bilaterally. In view of the clinical aspect, the diagnostic hypothesis was a melanotic macula associated with the adverse effect of ART, which it was confirmed through incisional biopsy. Conclusion: The reported case and the literature studies analyzed certify the need for greater knowledge of the factors associated with this oral finding in patients living with HIV, aiming to provide a better diagnosis. Introdução: A hiperpigmentação melânica da mucosa oral (HMMO) caracteriza-se pela deposição de pigmentos nos tecidos, decorrente do aumento da produção de melanina e/ou aumento do número de melanócitos. Essa manifestação clínica é um dos achados orais mais frequentes associados ao vírus da imunodeficiência humana (HIV). Alguns fatores podem contribuir para o seu desenvolvimento, tais como a terapia antirretroviral (TARV), a ação de certas drogas antifúngicas e antibacterianas e desregulação do sistema imunológico. O presente trabalho tem como objetivo explorar, através de um relato de caso e revisão na literatura, os possíveis fatores associados a maior predisposição da HMMO em pacientes com síndrome da imunodeficiência humana (AIDS). Relato de caso: Paciente do sexo masculino, 55 anos, vivendo com HIV em tratamento antirretroviral, apresentou-se ao serviço de Estomatologia da Universidade Federal do Rio Grande do Norte (UFRN) com queixa principal de “mancha roxa na bochecha” sem histórico de sintomatologia dolorosa. Ao exame clínico intraoral, foi observada a presença de duas extensas máculas em mucosa jugal, bilateralmente. Diante do aspecto clínico, a hipótese diagnóstica foi de mácula melanótica associada ao efeito adverso do TARV, confirmada por meio da biópsia incisional. Conclusão: O caso relatado e os estudos da literatura analisados certificam a necessidade de um maior conhecimento dos fatores associados a esse achado oral em pacientes vivendo com HIV, objetivando propiciar um diagnóstico mais preciso.
